Job Requirements

Essential Duties and Responsibilities

  • Ensures orientation of new patients immediately upon admission.

  • Obtains history and presenting information as delineated in program policy.

  • Collaboratively develops and updates an individualized treatment plan with the individual that includes targeted discharge date and plan.

  • Defines all relevant community sources affecting care and treatment of the consumer (e.g. attorney, family, case manager, etc.) and develops and follows contact plans and conjoint therapy plans.

  • Updates referring sources on patient progress, if/as approved by the individual in services.

  • Utilizes agency endorsed patient-centered therapeutic interventions and rapport-building practices, including motivational interviewing techniques, to non-confrontationally enhance patient insight regarding diagnostic and usage patterns.

  • Documents treatment episodes, contacts, treatment plans and sessions according to periodicity and content requirements.

  • Maintains appointed continuous caseload of actively engaged individuals.

  • Responds to crisis as needed.

  • Coordinates comprehensive discharge plan to ensure seamless community reintegration.

  • Performs all other duties as required or assigned.

Knowledge, Skills and Abilities

  • Ability to inspire client confidence in the FRN Staff and the FRN Model of Treatment.

  • Ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through the FRN Model of Treatment.

  • Excellent verbal and written communication skills required.

  • Excellent organizational skills with the ability to prioritize workload and meet deadlines.

  • Must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands.

  • Exceptional analytical and problem solving skills.

  • Proven ability to handle multiple projects under pressure.

  • Must have high level of interpersonal skills. Position continually requires demonstrated poise, professionalism, tact and diplomacy.

  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).

  • Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.

  • Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts.

  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources.

  • Knowledge of case management for dual diagnosis patients.

Education and Experience

  • Master's degree in counseling, psychology, social work, or related field required.

  • Licensure in a mental health related field is also required.

  • Minimum 2 years of experience in mental health or substance abuse field preferred.

The MSW will address the social-emotional problems, resource deficits, and care needs of the dying patients and their families served by the Hospice, assisting them to adapt and cope with the social and emotional impact of their terminal disease. The MSW is integrally involved in the informed consent process. The MSW will also counsel and assist in making funeral arrangements and provide the necessary resources pertaining to bereavement. ESSENTIAL DUTIES: * Follows established HIPAA Privacy Procedures when using and/or disclosing protected health information. * Complete and comprehensive assessment of patient and family needs to identify issues and determine support systems appropriate for each individual Hospice event. * Complete a psycho/social history of the patients behavioral/cognitive functioning, emotional/mental status, and adjustment to terminal illness. * Complete an assessment of patient's support systems, finances, living situation, legal concerns including advanced directive and patient preferences regarding end of life issues. * Responsible for initiation of bereavement services, making necessary contacts to appropriate resources. * Integral part of Interdisciplinary Team making necessary recommendations and participating in the development of the plan of care. Participates in the Interdisciplinary Team in developing a plan of care and making recommendations. * Provides individual or group counseling to help dying patients and their families adapt to the change related to their terminal disease by: Addressing any and all issues to assist patient and family to come to closure; Developing short and long term plan of intervention as needed; Addressing funeral arrangements or memorial service issues and concerns. * Providing emotional support to help patients cope. * Assists patients and families with access ot appropriate resources and services based on thorough knowledge of community resources and eligibility requirements. * Assists patient and families in completing applications for financial entitlement. * Assists with transition from home to Hospice House. * Assists with discharge planning if needed and facilitates transition back home. * Assists with informed consents and admission process to the Hospice as needed. * Participates in the quality improvement activities that pertain to the social work services for Sparrow Hospice Services. * Responsible for supervision and planning, organizing and directing the activities of medical social work services for Sparrow Hospice Services. * Prepares and provides in-services regarding Hospice psycho/social issues and care for Sparrow Hospice Services and the community when needed. * Is able to demonstrate excellent communication skills with patients, families, and members of Home Hospice Interdisciplinary Team in a professional diplomatic and empathetic manner. Able to provide clear, concise and timely written documentation on the patient's medical record and departmental record. * Promote the Hospice philosophy and purpose within the Health System and the community. Job Requirements General Requirements • Licensed in Michigan Work Experience • Minimum of one-year experience in health care setting and two year experience in Hospice related work.
