Clinical/Technical: Identifies daily patient assignment through hospital referral system and morning report, Conduct's age appropriate assessments pertaining to the psychosocial, economic, of high- risk medical patients. (List of high risk criteria in Case Management Policy and Procedure Manual) Provides further age appropriate psychosocial assessment, identifying functional, economic, cultural, and safety needs while assisting in establishing a treatment/care plan when requested by the patient, physician, or nursing services through use of health assessment triggers. (E.g. domestic violence, elder and child abuse/ neglect or substance abuse, suicidal, homicidal behavior, extended care placements, code status discussions etc). Communicates patient needs with patient and /or family to gain cooperation and involvement with treatment. Provides education, recommendation, and/or counseling pertaining to diagnosis and community resources. Assists the patient (and family) along with health care providers in establishing a safe discharge plan following patient's rights and regulatory requirements (CMS, HIPAA, State, and STANDARDS FOR ACCREDITING BODY ) for discharge planning. Advocates for patient rights. Serves as a member of the Medical Ethics Committee. Provides education to patients, staff and public on self- determination issues. Assists and educates unwed mothers on Ohio paternity issues. Schedules treatment room procedures. Records patient evaluation and progress information in the medical record. Actively communicates pertinent patient information to other members of the health care team; communicates with physicians; interacts with ancillary services; coordinates, collaborates, and functions as a team member with other providers of care to deliver effective continuous patient care. Performs pre admission and concurrent review for medical necessity, appropriateness of admission and level of care for Medicare and Medicaid inpatient admissions. Communicates concerns to RN case manger and physician. Works with the patient family and health care team to ensure safe discharge to appropriate level of care. Assists the RN case manager in creating and delivering preadmission and concurrent stay denials. (HINN and NOMAR letters.)

Supervisory: Directs and functionally supervise the activity of case manager assistant, Mentors and precepts new LSW to hospital environment and case management position.

Critical Thinking: Actively looks for and creates opportunities to improve department, staff and personal development, Develops and demonstrates knowledge of current developments in field to maintain professional competency. Utilizes hospital and departmental policies and procedures in patient care delivery.

Actively participates in Case Conference and Plan of Care with input from Patient, Patient's family and other members for the Health Care Delivery Team. Assess need of patient for potential referral sources of skilled services and community resources. Acts as a resource for new employees and precepts as needed.

Demonstrates a positive attitude, Provides excellent customer service, facilitates quality care delivery and fosters an atmosphere of understanding cultural diversity. Communicates dissatisfaction with issues to Director; actively contributes to the solution of problems and refrain from promoting dissatisfaction among co-workers.

Must be a team player, cooperate with co worker resolve conflict through one on one negotiation or with the assistance of the Director or their designee, Participates in providing in-services to department as needed. Maintains confidentiality at all times to protect patient's privacy and maintains HIPAA privacy and security regulations. Follows Social Worker Code of Ethics.

Documentation: Documents accurately (painting a complete picture providing directions to other health care providers) refrains from using unapproved abbreviations in written or computer documentation.

Documentation is completed on a daily basis. Understands the importance of accurate completion of documentation and standards for accrediting body requirements and how it impacts hospital payment and certification programs. Maintains accurate records. (MIDAS +, Time cards, on call log sheets, etc.) Unit Financial Accountability:

Maintains appropriateness of supplies, is accountable for productivity and management of time, Understands hospitals goals for financial viability. Education: Attends all mandatory education and in-services required for facility, Completes initial orientation and competencies per Human Resources established guidelines, Completes annual competencies and unit specific competencies per Human Resources established guidelines, Responsible and accountable for maintaining own state required CEU's per licensing board requirements. HIPAA privacy regulations and related procedures, CMS updates on benefit coverage and acute care policies, Review case management literature as distributed by Director.

Mandatory BLS requirements. Completes mandatory health requirements (e.g. annual TB testing, Fit testing and physicals as indicted. Attendance: Demonstrates regular and predictable attendance.

Work scheduled holidays. Overtime to be pre-approved per Director. Requested scheduled time off to be preapproved by the Director.

Performs other duties as required or assigned; adheres to facility dress code; assists in completion of special projects as required and /or assigned; responds to emergencies outside normal working hours as needed for Disasters and HECIS. Attends all mandatory education and in-services (i.e., team training, safety, infection control, etc.); completes mandatory health requirements. Employee performs within the prescribed limits of the hospital's and department's Ethics and Compliance program and is responsible to detect, observe and report compliance variances to their immediate supervisor, or upward through the chain of command, the Compliance Officer, or the hospital hotline.

Requirements

Completion of an accredited school of social work with current active registration in the State of Ohio with good standing. Masters in Social Work, Licensed as an Independent Social worker in the State of Ohio; completes 30 contact hours of approved CE during each two year renewal period consistent with requirements of the Ohio Board of Social Work. Maintains continuous certification in American Heart Association's BLS.

Shift

1st (Monday - Friday) w/ weekend and evening call requirements 1 -2 times per month
