As a licensed nurse at our Community, you will assist with the comprehensive delivery of personal care services to residents on a day-to-day basis following required regulatory requirements. You will also contribute to maintaining a physical, social and psychological environment, which will be conducive to the best interests and welfare of the residents.

Minimum eligibility requirements

  • Licensed Registered Nurse with current North Carolina State Licensing Authority in good standing with the North Carolina Board of Nursing.

  • Experience of 1-3 years or more in the care of senior adults and adults with cognitive decline is preferred.

  • Must have a working knowledge of Rules 10A NCAC 13F for Adult Care Homes relevant to LHPS nurse duties.

  • Must be able to communicate effectively with residents, families, staff, community and State officials and the public.

  • Must have compassion for and desire to work with the elderly.

  • Must demonstrate the ability to work responsibly as a team member as well as an individual.

  • Must be honest, fair, dependable, respect confidentiality and the rights and privacy of others.

  • Practice and promote Affinity Policies and Procedures, Mission Statement, Core Values and Founding Principles.

  • Must have exceptional organizational skills and ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the resident and Community.

  • Ability to read, write and speak English.

  • Must have strong documentation skills.

  • Must pass criminal background check.

  • Must meet all health requirements.

  • Must have valid driver's license.

  • Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodation.

Essential functions

  • Ensure sound infection control practices are in place that promotes a healthy environment

  • Contribute to resident assessments and person centered care and service plan development.

  • Perform medication pass observations in conjunction with regulatory and community compliance requirements.

  • Provide competency validation for all care staff providing resident care

  • Provide Licensed Health Care Professional quarterly assessments for residents identified by community management.

  • Administer and read the two-step tuberculin skin testing as required. (Must have doctor's order for both staff and residents to administer).

  • This role does not include the performance of direct resident care, or performance of procedures: including IM injections, blood draws, treatments, foot care, or medication administration.

  • Provide ongoing oversight and certification for medication aide training including medication aide skills check to meet state regulatory requirements and review of additional 5-hour and 10-hour medication training with sign off certification and review.

  • Monitor staff completion and review of E-learning online medication aide, Diabetic training to include finger stick blood sugar monitoring administration of insulin (prior to administering insulin as per regulations , Coumadin, and 3 hour State required infection control trainings with certification prior to providing skills check-offs for staff.

  • Provide Licensed Health Professional Support (LHPS) skill checks to assure competency of personal care staff to meet regulatory requirements.

  • Provide other training/in-services to medication aide and personal care staff relevant to resident care as requested by the facility management and within the Registered Nurses' scope of practice according to the North Carolina Board of Nursing

  • Participate in resident and family care conferences as requested by facility management

  • Maintain all privacy rules and regulations pertaining to the Health Insurance Portability and Accountability Act (HIPPA).

  • Assess for changes in resident condition based on quarterly assessments and or significant changes to ensure timely evaluation, monitoring, documentation in assisting the facility to providing resident care.

  • Other duties assigned by Resident Care Manager, Memory Care Manager, and/ or Executive Director within the LHPS role and scope of practice.

Essential functions Personal care and health care: * Assist residents with activities of daily living, including bathing, dressing, grooming (oral care), toileting, changing of bed linens, positioning, transfer, mobility, and incontinence care. Encourage residents to be as independent as capable. * Keep proper records of care as required by the facility. Complete all forms and documentation as required. * Immediately report any observations of concern/changes in resident's condition to the Supervisor in Charge. * Vital sign monitoring will be performed as needed or directed by regulations, policy or primary care provider's order. * Be able to apply the home's accident, disaster, fire safety, elopement and emergency procedures for protection of the residents. * Be familiar with Affinity's State Regulatory Policies and Procedures. * Be capable of performing CPR upon successful completion of CPR certification. * Be knowledgeable of the residents' background, preferences, habits and interests and preserve confidential information about each resident. * Focus on service to residents in during dining and promote meal time as an enjoyable activity of each resident's day. * Provide assistance to individual residents as needed or directed, who require help with eating, carrying plates, trays or beverages. Safety and Sanitation * Monitor all exit doors, the call and alarm systems to assure prompt response. * Understand fire watch and need to check exit doors during and after as directed to assure full closure and lock. * Maintain the confidentiality at all times of all mag-lock code systems to assure safety of the residents. * Promptly answer facility and common residents' telephone in a professional manner and per Affinity policy in order to meet the needs of the caller and communicate information. * Understand and follow safety and infection control policies and procedures and readily report any breach of the safety and infection control procedures to the Supervisor In Charge. * Respond to emergency call signals promptly. * Report incidents to the Supervisor in Charge promptly and complete appropriate paperwork as required in a professional manner. * Readily and directly report any verbal or physical abuse of a resident, inappropriate use of resident or facility property or lack of or inappropriate care of a resident to the Supervisor in Charge, RCM or Executive Director if necessary. Laundry/Cleaning Functions * Collect, laundry, and distribute linens, garments, etc. in accordance and as permitted by State regulations and with Affinity procedures. * Help to properly maintain all equipment and ensure work areas are clean and safe. * Promptly notify the Supervisor in Charge of items that need repair or present a safety risk to residents, employees or visitors. * Assist in cleaning resident rooms, dining areas, public areas as indicated by regulations. * Notify housekeeping of spills, soiled areas and assist with clean up as per State Regulations as needed.
