The Institute of Living, one of the first mental health centers in the U.S., remains one of America's leading not-for-profit centers for patient care, research and education in the fields of behavioral, psychiatric and addiction disorder. Through our dedicated, experienced and skilled clinicians, we provide patients and their families with the highest quality of personalized care. This position is a collaboration between the Institute of Living and Hartford HealthCare's Ayer Neuroscience Institute, and reflects our commitment to embedding behavioral health services in medical settings throughout the system.
This Clinician will provide quality, cost-effective outpatient care at the Multiple Sclerosis Center in Norwich, CT for patients and/or their families by assuring access, providing assessment, coordinating required resources, and partnering with health care team members. Duties include working in an integrated care setting to provide behavioral health services to an adult population with MS and other immune related diseases of the nervous system. The clinician will primarily support MS patients with additional support to be provided to headache and epilepsy patients on an as needed basis. Specific duties include diagnostic assessment and evaluation of behavioral health treatment needs of medical patients on site, collaboration and consultation with medical providers, brief and evidence-based treatments for behavioral and mental health issues, effective utilization of screening measures and assessment tools, development and implementation of group programming, data management including tracking symptoms and treatment outcomes, participating in program development, developing and implementing educational materials for use in the medical setting, participating in team meetings, case management, outreach and communication with outside providers, and independent maintenance of treatment records. The clinician also plays a role in helping with resource coordination for those that face barriers such as living situation, family support, and other obstacles. By utilizing excellent communication and negotiation skills, focuses on providing the appropriate service in the most cost-effective formula to assure that the patient, provider, payor and health organization's goals are met. While the clinician is fully embedded in the Multiple Sclerosis Center, the position is clinically and administratively supported by the Division of Health Psychology at the Institute of Living.
LCSW licensed to practice in the state of CT
Ability to meet requirements for internal credentialing in individual and group or family therapy required.
Demonstrates competencies, including knowledge and behaviors, to meet age specific patient needs.
