Specific Responsibilities:

  • Will supervise a staff of Social Workers and Client Mentors across multiple locations.

  • Provide intensive case management services with a diverse population of adults, clients involved with Welfare to Work activities; Complete psychosocial assessments to identify strengths, barriers, and service needs of clients and families.

  • Collaborate with PCG team members, County case workers, behavioral health providers, and other relevant parties.

  • Assist in development of care plans mitigate barriers self-sufficiency.

  • Participate in regularly-scheduled case conferences involving Senior Mentors, ETAs, and other team members;

  • Document and track assessment results, case actions, and client progress;

  • Work effectively with Program Coordinator and team of Senior Mentors to strengthen families and provide them with coping skills as they prepare to enter/return to the workforce.

  • Supervise clinical interventions of Senior Mentors.


Required Skills:

  • Experience completing psychosocial assessments and providing case management with diverse populations.

  • Demonstrated ability to establish and execute defined program goals and objectives to ensure compliance measured performance.

  • Superior organizational and interpersonal skills.

  • Experience engaging and maintaining relationships with community partners serving program participants.

  • Exceptional strength in strategic thinking, development of effective action plans, and problem solving.

  • Superior verbal and communication skills.

  • Ability to recognize and maintain the confidentiality of all materials in the work setting.

  • Ability to work independently, requiring minimal supervision.

Required Education and Experience:

  • Licensure by the State of California as a LCSW

  • A minimum of a Master's Degree from an accredited U.S. college or university, or a certified foreign studies equivalency in counseling, social work, or related field;

  • Proficient with MS Office: Word, Excel, Outlook, PowerPoint

Social Workers provide an important role within the hospital by assisting patient and families with personal and environmental problems which predispose illness or which interfere with their obtaining maximum benefits from medical care. Provides peer supervision for other staff, education for social work interns and/or assists with program planning and organization of services. Demonstrates ability to perform competencies as outlined in unit specific competency assessment tools. Demonstrates age specific care as established for a Clinical Social Worker in assigned area. This is an 8 Hour, Part-Time (32 hours per pay period), EVENING shift position, with varied days that may include weekends and holidays. This position will be assigned to the Emergency Department. Required Qualifications: * Master's Degree in Social Work from an accredited university or college (MSW) * American Heart Association BLS for Health Care Provider * Must be able to display good working relationships with patients, families, medical and nursing personnel, and all others employed at the hospital. * Working knowledge of the laws, regulations, policies and appeal procedures as relative to the Federal, State and other reimbursement programs. Preferred Qualifications: * Acute care hospital experience * Licensed by the State of California Behavioral Science Examiners Board as a Licensed Clinical Social Worker (LCSW) preferred.
