Job Overview:

We are seeking a compassionate and qualified Licensed Clinical Social Worker (LCSW) to join our team and provide exceptional therapeutic services to our clients in the comfort of their own homes. The LCSW will work collaboratively with a team of home care professionals to deliver evidence-based interventions and support clients in achieving their treatment goals while remaining independent at home.

What you'll do:

• Conduct comprehensive assessments to diagnose mental health conditions within the home setting.
• Develop individualized treatment plans and provide psychotherapy to home-bound clients.
• Utilize a variety of evidence-based therapeutic techniques appropriate for the home environment.
• Collaborate with other home care professionals, medical providers, and family members to ensure coordinated care.
• Document treatment progress and maintain accurate client records.
• Participate in ongoing professional development activities.
• May provide crisis intervention and support services within the home setting.

The Ideal Candidate:

• Master's Degree in Social Work (MSW) from an accredited program.
• Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
• Minimum of 5 years of experience working with clients in a home care setting or related field.
• Experience providing individual therapy to adults [or children/adolescents/families if applicable] it the home environment.
• Strong clinical skills including assessment, diagnosis, treatment planning, and intervention.
• Proficiency in evidence-based therapeutic modalities (e.g., CBT, DBT, etc.).
• Excellent communication, collaboration, and interpersonal skills.
• Strong documentation skills.
• Ability to work independently and as part of a team.
