This Ka Makana Alii Clinic features team-based adult and pediatric primary care to consistently manage the overall health care needs for patients of all ages as well as Obstetrics, Gynecology and family planning services. Pediatric Sports Medicine focuses on prevention and treatment of fitness activity injuries. Specialty Care is provided at the clinic by rotating Hawai'i Pacific Health Specialists or in coordination with over 1,800 system Physicians and all services of Kapi'olani, Pali Momi, and Straub Medical Centers.

On-site, walk-in, laboratory and X-ray imaging bring diagnostic services to this neighborhood. Supported by the most advanced electronic medical record system the clinic's team leverages the MyChart patient portal which engages patients to connect with their doctor and care team, manage their health, optimize their appointments, and help family member track their health.

If you are compassionate and empathetic, you could be our next Behaviorist to provide invaluable integrated behavioral health services to patients currently receiving primary care. You would be the main interface for patients to access services such as peer/family counseling and support, screening, assessment, brief treatment, consultative services, education and engagement, linkages, outreach and advocacy. We are looking for someone sensitive and perceptive, with a strong sense of community care and social service, who shares our passion for health care in the community.

Minimum Qualifications:
Master's Degree in Social Work.

Current Hawai'i Clinical Social Work (LCSW) license. Current American Heart Association Basic Life Support (BLS) card. Professional knowledge of loss, ilness, and disability issues.

Preferred Qualifications:
Two (2) years of experience in a LCSW role.

Duties Summary Social Workers seek to improve the overall quality of life of people within the context of their environment through the remediation and prevention of their problems, and by improving accessibility, accountability and coordination of service delivery among a variety of professionals and/or agencies tasked with providing appropriate services to clients. These professionals come from a variety of interdisciplinary bases, establish working relationships of mutual respect and trust with the clientele served, and utilize a variety of helping interventions to enable these clients to function as effectively as possible. These positions are found in a variety of settings and work with diverse types of people. They may be found in hospitals and other health service agencies, social service agencies, schools, correctional facilities, and programs concerned with substance abuse, aging, and family violence. The Social Worker II is the advanced trainee level in this class. Positions in this class continue to receive training in the principles, practices, and techniques of professional human services work and are assigned increasingly complex cases while continuing to advance toward becoming independent workers. PROGRAM PREFERENCE CHOICES: The following is a brief description of the different programs for the Human Services Professional within the Department of Public Safety: Adult Corrections (AC) - Counsel and work with the adult inmate population, assisting them in adjustment to prison life as well as rehabilitative/redirection services and transition back into the community. Intake Services (IS) - provides casework services to assess felony and misdemeanant offenders for release alternatives pending trial, supervises pre-trial and pre-sentence felony and misdemeanant offenders as ordered by the courts, provides and arranges for social welfare and other services to assist offenders in the community; recommends security classification levels for offenders admitted into the community correctional center; maintains operation of the electronic monitoring program for eligible pre-trial and sentenced offenders; and implements the Community Service sentencing program for the courts. Minimum Qualification Requirements To qualify, you must meet all of the following requirements: Basic Education: In accordance with Act 238, SLH 2005, applicants must possess a bachelor's or master's degree in social work from a program of study accredited by the Council on Social Work Education, or a doctoral degree in social work from a college or university accredited by the Western Association of Schools and Colleges, or a comparable regional accreditation body. Specialized Experience: No Specialized Experience required. Note: Applicants who will graduate within six months, and who will meet the Minimum Qualification Requirements upon their graduation will be accepted for examination. If you are graduating within six months with a Bachelor of Social Work degree or applicable Master's degree, please submit a copy of your official transcript of coursework completed thus far and a letter from the registrar indicating your degree type and anticipated date of graduation. Driver's License: A valid driver's license is required for most positions. For those positions requiring a driver's license, you must possess a valid driver's license at the time of appointment.
