40 hours/week Concord, Adult Primary Care and Behavioral Health Integration Concord, NH

Point person for promoting, providing, and coordinating high-quality behavioral healthcare as a member of the primary care team.

Responsibilities:

  • As a member of the primary care team, help address the behavioral health needs of patients in primary care utilizing the collaborative care model of integrated care.

  • At the request of the PCP or in other appropriate situations, conduct a triage visit or initial evaluation to help clarify behavioral health concerns and develop an individualized, appropriate care plan.

  • Enroll patients referred by the PCP into the collaborative care program which includes both in person and telephone outreach visits for a short-term episode of care, up to 12 weeks duration.

  • Utilize standardized, evidence-based clinical measures to assess patient's symptoms and response to treatment.

  • Proactively communicate with patients, primary care team members and the consulting psychiatrist to aid in effective, patient-centered, team-based care.

  • For carefully selected patients, provide face-to-face, short term counseling including; problem-solving therapy, brief CBT, motivational interviewing or brief supportive therapy (average 6 sessions per patient, per episode of care).

  • Partner with PCP teams to systematically screen and assess for opiate misuse and other substance use disorders.

  • For carefully selected patients, partner with PCP teams to provide team-based treatment for opiate and other substance use disorders in primary care.

  • Assist patients in need of higher levels or care, specialty mental health or substance use services, outside of primary care, to connect with appropriate treatment resources.

  • Effectively maintain and manage a patient registry.

  • Performs other duties as required or assigned.

Minimum Qualifications:

  • Master's degree in social work from an accredited institution of higher education, including an internship meeting professional standards.

  • Must be able to demonstrate that he or she has provided psychological services, on an inpatient, outpatient or consultative basis in the past 12 months.

Functions: 1. Provide AT evaluation and consultation services, which includes AAC, switch and/or computer access. 2. When necessary and according to policy, implement infection control procedures such as cleaning equipment, disposing of hazardous waste, and cleaning spills of bodily fluids. 3. As a member of the ATECH Team, participate in Team Meetings, collaborate with colleagues and contribute to specific projects. 12. Provide services according to accepted Standards of Practice, ATECH/NH-ATEC Policies and Procedures and Professional Codes of Ethics. 13. In addition to providing quality services, integrate customer service and cost containment strategies into work activities. 14. Represent ATECH Services at public events. 15. Collaborate with staff from other branches of the organization 16. Conduct training activities as assigned. 17. Conduct research activities as assigned. 18. Other duties as assigned. Requirements: 1. A Masters degree in Speech-Language Pathology. 2. Holds Certificate of clinical competence (CCC-SLP) granted by ASHA; or meets requirements for CCC and is completing the 9 month CFY required by ASHA. 3.
