Responsibilities

Below is a list of your responsibilities as an Licensed Clinical Social Worker with Corizon Health at Polk County Central County Jail located in Bartow, Florida:

  • Provides direct clinical, mental health treatment and consultation services in accordance with policies, procedures and regulations of state-specific statutes, policies and regulations of the facility and in accordance with the ethics and standards of the National Association of Social Workers

  • Ensures multidisciplinary treatment planning that guides services provided

  • Provides crisis intervention as required and implements Corizon's Suicide Prevention Program

  • Acts as a liaison between corrections and the local community mental health agencies.

  • Interrelates and works effectively with other mental health services staff, facility administration and staff, healthcare staff, and outside support agencies as designated by the facility

  • Maintains confidentiality of patient information in accordance and within the limits of the standards of the American Psychological Association, the National Commission on Correctional Health Care, the American Correctional Association, and the American Psychiatric Association

  • Utilizes the SOAP charting format to document the document mental health services provided to the patient

  • Complies with all rules, directives, policies, procedures and protocols of Corizon

  • Other duties as assigned

Experience & Requirements

  • Master's degree in social work from an accredited program

  • Licensed Clinical Social Worker

  • Current certification in CPR or ACLS

  • Professional experience, which indicates knowledge of mental health counseling, group and individual psychotherapy, treatment of major mental disorders and mental health assessment techniques, preferably in a correctional setting

  • Required to exert up to 20 pounds of force to lift, carry, push, pull, or otherwise move objects.

  • Required to kneel, stoop, crouch and/or crawl occasionally.

  • Occasionally walking throughout site; to include walking up and down flights of stairs.

  • Must be able to perform repetitive tasks using arms, hands, and wrists.

  • Long periods sitting, typing and reading from a computer screen may be required.

A background check, security clearance and drug screening are required for this role.

This position is for a Licensed Practical Nurse at the Lakeland clinic, whose priority functions will be (1) Providing childhood/adult immunizations 2) Maintain proper storage and handling of vaccines per Health Department policy and procedures 3) Provide counseling and teaching information regarding administered vaccines to client and/or client's parent/legal guardian. 4) Assist in pediatric clinic to provide services to clients under the direction of a Registered Nurse, licensed Advanced Registered Nurse Practitioner, and/or Physician in the Pediatric clinic settings; IMMUNIZATIONS: * Provide assessment and administration of immunizations, according to CDC and VFC protocol for pediatric clients and adult clients * Provide counseling and teaching information regarding administered vaccines to client and/or client's parent/legal guardian. * Maintain proper storage and handling of vaccines per Health Department policy and procedures. * Accurately complete "Immunization Clinic Record/Signature Card' and all other documentation required per Health Department policy and procedures for vaccines administered. * Entry of immunization history and immunizations given into Florida Shots. * Order vaccines as needed, check expiration dates on vaccines and rotate as needed * Responsible for monitoring temperature controls with notation on appropriate logs for refrigerator and freezer per protocol. * Handling, ordering, inventory and usage monitoring in accordance with Department of Health Policy 350-8-12. * Serves as a resource leader for public and private sector vaccine providers (i.e., answers questions concerning vaccine recommendations, schedules, administration along with inventory and storage and handling issues) to improve immunization rates. * Promotes educational materials concerning vaccine and administration. CLINIC * Interview and assist providers with pediatric clients with specific medical problems or routine health care needs. * Assist ARNP and physician with examinations and medical procedures for clients. * Perform routine diagnostic tests, procedures such as taking/recording temperature, pulse, respiration and blood pressure; collecting specimens for laboratory analysis. * Report any unusual symptoms of clients to clinic supervisor, ARNP, and/or physician. * Provide and maintain accurate documentation in client electronic records in accordance with Health Department processes, protocols and standards. * Provides medications according to the provider's orders. Maintains the medication inventory. * Assists with case management of clients and make referrals for clients needing follow-up treatment. * Clean work area according to clinic policy and biohazardous waste policy. * Provide medication to clients according to nursing policies and protocols. * May direct activities of other employees as instructed by the clinic supervisor. * Participate in orientation and staff development activities. * Conduct inventory of medical supplies.
