What You Will Do Provides behavioral health counseling services. Conduct patient assessment and establishe therapy treatment goals. Administer individual and group therapy.
Maintains thorough and confidential records of observations. Evaluate progress and effectiveness of therapy interventions. Provide patient with information regarding resources and therapeutic activities in the community.
Work directly with the patient, family, physician and community providers to ensure active and collaborate care from the initiation of treatment to the time of discharge. What You Will Need Licensure / Certification / Registration: One or more of the following:
Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date required. Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date required.
Social Worker specializing in Clinical Practice credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date required.
Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date required. Therapist specializing in Marriage & Family credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date required. BLS Provider credentialed from the American Heart Association (AHA) preferred.
Supervisor credentialed from the Wisconsin Department of Safety and Professional Services preferred. Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required. Additional Preferences LCSW with experience Strongly Preferred.
