  • Per Diem, Days. Various hours a week.# *Includes Per Diem Differential. +$2.50 for weekend shifts.# Boulder Community Health is seeking an Addiction Counselor who develops and implements a culturally responsive, evidence-based substance use disorder (SUD) treatment program, who performs assessment, therapy, and treatment planning.

Responsibilities #Develop and implement evidence-based SUD therapy program. #Mentor and educate staff in SUD assessment, treatment, and recovery. #Ongoing monitoring to assure program meets statutory, regulatory, compliance, and quality requirements. #Data-driven incremental improvement. #Assess patient#s condition and identify problems, goals, and intervention. #Collaborate with patient, family or loved ones, and interdisciplinary team to develop and update treatment plan. #Act as patient advocate. #Ensure care is trauma-informed and appropriate to patient#s emotional, social, and mental development. #Facilitate group therapy and individual therapy sessions.# #Educate and validate patient, family and/or loved ones# understanding.
