Essential Duties And Responsibilities

As our Medical Director, you will work closely with the Market Medical Director to play a critical role in both care delivery and ongoing practice innovation. Specific areas of focus include:
Providing primary care within the practice, including face-to-face visits as well as non-visit based interactions (e.g. phone, email, groups, home visits, video chat)
Comfort working with a medically complex population
Supervising and teaching a team of health coaches, who, based on a community health worker or promotora model, will build strong relationships with patients, help patients set self-management goals, improve self-efficacy, and support clinical work
Evolving the way we care for our patients through creative innovation of care delivery; contributing to practice improvement activities and across our practice network
Communicating and marketing the practice model to new patients as well as other stakeholders such as the medical community
Developing a medical network with trusted outside medical consultants, hospitalists, emergency room providers, support services and others
Qualifications
We seek a board-certified/eligible Internal Medicine, Family Medicine, or Geriatrics trained physician who has or is able to obtain Arizona state licensure.
The ideal candidate will be an excellent clinician, a systems thinker, technologically savvy, excited to work in an interdisciplinary team, value service to our patients, and revel in uncertainty and ambiguity.
The ability to build successful relationships with team members (who have a wide range of professional training) by working without ego and with strong collaborative spirit is an essential attribute.
Candidates with work experience outside of medicine, and those with other advanced degrees or specialization, are encouraged to apply, as are recent residency program graduates.
Spanish language fluency highly preferred but not required

The Associate Pharmacy Care Coordinator will be part of a call center team responsible for direct communication with team members, patients and health care providers to ensure prompt processing and delivery of prescriptions. The Associate Pharmacy Care Coordinator will need to manage both people and processes ensuring that service levels are achieved and internal as well as external customer expectations are not only met, but surpassed on a consistent basis. This position requires superior customer service skills, exemplary communication skills, critical thinking and problem solving skills. Primary Responsibilities: * Promptly and accurately does benefits investigations * Highlight any important notes / requests from the provider in the case * Prioritize and organize cases to be processed in timely manner proportionate with the need by date * Accurately enter orders into the pharmacy system * Verify validity of prescriptions and documents verification per company procedures * Contact the physician's office to communicate clinical information when needed * Notify pharmacist of relevant clinical information gathered during calls to provider or patient that may affect the patient's decease state or medication regimen * Verify secondary insurance with each prescription that requires billing a secondary plan * Forward cases to Financial Assistance Team if financial assistance is needed * Notify Pharmacist to transfer prescriptions that can no longer be filled at AOS / Avella to appropriate pharmacy along with notifying the provider and the patient * Follow up with transfer until a resolution is reached * Work with physician offices to facilitate the process of obtaining prior authorization from third party payers in order to maximize patient's access to treatment * Monitor, follow up with, and record all pending insurance prior authorizations to expedite processing time * Contact patients to arrange delivery of medication, verifies delivery address and collect payment information * Offer the patient a consultation from a pharmacist * Answer telephones and take messages for patient refill requests and related pharmacy business * Check voicemails on assigned days * Report all customer complaints to a supervisor and allows the supervisor to rectify the situation with the complainant * Help to maintain the pharmacy in a clean and orderly condition * Provide excellent customer service to all patients and medical professionals via phone, written communication or in person * Maintain a collaborative approach with members of the Avella team * Demonstrate the utmost sensitivity and confidentiality of information while maintaining a high level of professionalism * Assume additional projects on an as needed basis Required Qualifications: * High School Diploma / GED (or higher) * 6 months of pharmacy related experience, including retail pharmacy experience * National and State Pharmacy Technician licensing and registration in Arizona * Availability for an 8 - hour shift between 6:00 am - 6:00 pm Monday through Friday with rotating weekends OptumRx is an empowering place for people with the flexibility to help create change.
