Lead Registered Nurse

Status: Exempt

Reports To: Director of Health Services

HOPE Services Hawaii is seeking a Lead Registered Nurse (RN) to become part of a dynamic, interdisciplinary team. The Lead RN works with houseless individuals in our community, offering urgent and continuous medical and behavioral health care. HOPE Services Hawaii is committed to delivering high-quality nursing and care management in various non-traditional settings, including but not exclusive to outdoor community areas, homeless shelters, and resource centers. The role requires a professional who is flexible, compassionate, self-motivated, and dedicated to our mission of advocacy and empowerment.

Essential Functions:

  • Observe strict compliance with Health Insurance Portability and Accountability Act (HIPAA) privacy and security rules for protected health information.

  • Provide skilled nursing at HOPE Shelters to assess, treat, and educate guests needing medical assistance according to licensure regulations and scope of practice.

  • Document all services provided per agency standards and participate in tracking and reporting activities to support effective program management.

  • Provide training to increase staff knowledge of health care issues impacting the target population and to promote effective communication and comprehensive care.

  • Engage individuals with substance use disorder and mental illness with a strong commitment to harm reduction principles and trauma-informed care.

  • Attend mandatory organizational meetings for staff, program, and organizational development.

  • Performs other duties as deemed necessary by supervisor.

Specific duties include but are not exclusive to:

  • Create and manage the care plan for program participants with chronic or advanced comorbid medical and mental health conditions.

  • Advocate for personalized treatment options that address a program participant's unique care needs.

  • Collaborate with the team to schedule medical appointments and follow up to ensure attendance.

  • Communicate health status information with the patient, family, caregivers, and community case managers.

  • Offer program participants education and guidance for navigating complex medical decisions.

  • Serve as a liaison between program participants and their insurance providers to promote quality, cost-effective care with the best program participant outcomes.

  • Perform physical, functional, cognitive, and psychosocial assessments of individuals.

  • Assist the Director of Health Services with medical interventions as defined by RN scope of practice.

  • Facilitate collaboration between the individual and the multidisciplinary team to increase program participant's capacity for independent medical and behavioral health self-management.

  • Identify and meet the program participant's need for health education or coaching, link the program participant with community resources as indicated, and utilize motivational interviewing techniques to empower meaningful choices as influenced by the service consumer.

  • Identify potential substance use disorders and/or mental illness and work with the Behavioral Health Team to follow up accordingly in consideration of the possible need for access to treatment resources including counseling, therapy, and recovery programs.

  • Support the program participant's right to refuse or revoke services and/or seek restitution or grievance when an individual feels services are inappropriate or inadequate.

  • Support laboratory monitoring of medication efficacy.

  • Provide support for ambulatory detox as assigned.

  • Establish achievable goals and interventions to be integrated into the individual's Master Individualized Recovery plan.

  • Perform continuous evaluation of an individual's response to care.

  • Assist the Director of Health Services in reviewing hospital and agency referrals to HOPE Services to determine placement in a medical respite program or in a congregate shelter bed.

  • Participate in developing healthcare protocols and staff training with the interdisciplinary team.

The Lead Registered Nurse must have knowledge and experience or be willing to undergo training in the areas of:

  • Environmental and personal healthcare safety regulations

  • Health care, mental health care, substance use, and addiction care and services

  • Legal requirements and risks to perform this type of work, including the relevant legislation that impacts decision-making

  • Rental housing and requirements of tenants and landlords by law

  • Homelessness, acute/episodic, and chronic

  • Culture diversity and cultural identity

  • Changes across the lifespan

  • Child welfare

  • Harm reduction

  • Trauma - physical, mental, emotional.

  • Fetal alcohol spectrum disorders

  • Privacy and confidentiality

  • Corrections and Criminal Justice

In the course of meeting Lead RN responsibilities, it is not uncommon to witness, engage in, or be confronted with the following: Violence and threats of violence; profane, racist, or sexist language; interactions with First Responders; alcohol-influenced behavior; street drugs; tobacco smoke; death of service participants or her/his associates; nudity of service participants or her/his associates; disruptive friends/family dynamics with service participants; people involved with sex work; people involved in the drug trade; persons performing actions against their consent, will or knowledge; people in conflict with the law; infection risks; and/or other situations that may be unsettling. Measures are taken to train staff to deal with these situations appropriately. Still, those in the position should reasonably expect these types of things to occur and understand appropriate direction and support will be provided by HOPE Services Hawaii leadership.

Supervisory Responsibility:

This position is responsible for the oversight of registered nurses within the organization. There may be some shared supervisory responsibility for students or interns completing a practicum and community volunteers providing project or event assistance. The Lead RN oversees safety, efficiency, and positive outcomes.

Minimum Qualifications:

  • Current, unencumbered State of Hawaii RN license.

  • Preferred two years of nursing experience.

  • Preferred one year of supervisory experience

  • Crisis Prevention (CPI) certified or able to certify through the organization

  • CPR plus AED certified or able to certify through the organization.

  • Current Hawaii driver's license, clean abstract with no major traffic violations within the last five years, and access to an insured vehicle for your use in conducting business.

Physical Demands:

The position requires adaptability and the ability to work under pressure. The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully. While performing the duties of this job, the employee is regularly required to talk and hear. The employee must frequently stand, walk, and use hands to reach. Occasionally lift, bend, stoop, push, and pull up to 50 pounds.

Scheduling Requirements:

Within the general business hours of 8:00 a.m. - to 4:30 p.m. Monday through Friday, this is a 40-hour-a-week, exempt position with flexible scheduling to meet client and program needs. You may be required to be on-call and/or work evenings, weekends, or holidays.

Salary starting range: $95,000.00/yr-$120,000.00/yr
