RESPONSIBILITIES

• The RN Lead demonstrates support of the Company's mission by providing care management/disease management services to clients and families to help in the coordination of care and the management of a patient's individual health needs through and beyond the course of treatment.

• The Lead RN demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient's plan of care while ensuring and facilitating the achievement of quality, clinical outcomes.

QUALIFICATIONS

• A valid Florida RN license or if reside out-of-state, a multi-state valid RN license.

• Bachelor's Degree in Nursing from an accredited college,

• OCN certification or Case Management certification preferred.

• Three (3) years of clinical oncology experience required.

• One (1)year of experience in Care Management and/or similar role preferred.

• Strong organizational and interpersonal skills required.

• Must possess the ability to utilize a clinical reasoning process for assessing, planning, implementing and evaluating the patient's plan of care while ensuring that coordination of services is done in a timely manner.

• Demonstrates sound knowledge and actions in the care and decision making for the oncology patient population and seeks guidance appropriately.

• Strong computer skills needed, Examples: Microsoft Office, Word, Excel and Outlook required.
