Responsibilities

GENERAL PURPOSE:

A critical member of the Division of General Pediatrics and GCH Pediatric Practice leadership teams, the Community Clinical Project Director leads planning, integration, and clinical operations for select community projects fulfilling the GCH Strategic Plan 'to do everything in our power to help each child reach their fullest potential', with particular emphasis on the Strategic Plan Pillars of Community and Advocacy, and Population Health and Partnerships, and focusing on health equity and evidence-directed impact. Responsible for the overall direction, coordination, implementation, execution, control and completion of multiple, simultaneous projects. Projects include (but are not limited to): expanding telemedicine capacity/use within Rochester City School District (RCSD) and the Rochester community to more meaningfully meet children and youth where they are, and further healthcare and behavioral health service provision/outcomes; integrating more thoroughly the relationships between the Department of Pediatrics and GCH, RCSD Student Health Services, and community pediatric-serving providers; and fostering relationships, partnerships, programs and engagement around developing the clinical dimensions of whole child/family health with community partners including government, public policy, education, human services and community based organizations. Guides others in resolving complex issues in own specialized areas and solves complex problems with implications on cross-functional processes and outcomes.

RESPONSIBILITIES:

Primary Golisano Children's Hospital liaison with Rochester City School District (RCSD) Student Health Services; provides direction, guidance and support to RCSD Coordinator of Student Health Services. Leads health and related idea generation and project transfer/initiation bi-directionally to develop relationships and better respond to student health needs through collaboration. Directs GCH promotion of student well-being through support of the RCSD Student Health Services program, to include professional school nurse development, workforce, and retention initiatives; as an example, apply ECHO model with school nurses for topics such as: telehealth; asthma, respiratory, and chronic condition care; medical system integration, etc. Develop school nurse connections, relationships with, and knowledge of/access to established professional nursing development resources.

Lead and further develop new methods for communication between school nurses and local medical system providers/clinicians beginning with RCSD student health services integration and Rochester RHIO work-in-progress focused on quality care in partnership with schools. Evaluate and elevate the role of/connection to school nurses within the Rochester community and child-serving services and systems. Participate in Connecting 4 Kids with a particular focus on enhanced communications between key stakeholders, including school and health personnel and families.

Facilitates the clinical aspect of community telemedicine adoption and expansion. Co-lead the development, refinement, and standardization of workflows/protocols and training related to school telehealth encounters, particularly in new services (mental health, developmental and behavioral pediatrics, and for chronic medical conditions) and new pediatric/family settings such as child care locations, Head Start and Universal Pre-K programs, etc.

Leads the development and coordination of newly undertaken, comprehensive, and community-wide asthma population health initiative. To include the practical and coordinated application of established evidence; interventions with demonstrated benefit including: access enabled by telemedicine; school-based preventive care; applying the Global INitiative for Asthma (GINA) strategy and (Single) Maintenance And Reliever Therapy [(S)MART] approach; and a Community Health Worker (CHW) model to address social/environmental determinants of health. This includes defining project scopes and goals, working with school districts, school health services, local primary and specialty care practices, pharmacists, Medicaid, and insurers. Apply relevant pediatric asthma care learnings to improve whole child/family health outcomes and address other chronic conditions over time.

Integrate clinical components of the EACH (Enhancing Access to Child Health) Youth in the RCSD initiative with Administrative and Financial Project Directors and Project PIs. Develop and define organizational functional structures and workflows. Serve as one of several clinical liaisons and aid in the evaluation component of the EACH Youth project.

Participate in strategic planning, development, and related policy/advocacy efforts. Assist EACH Youth PIs, Leaders, and Co-Directors identify challenges and needs to enhance functional structure as initiatives grow and move toward sustainability. Co-represents EACH Youth on NYS Medicaid 1115 Waiver implementation for Rochester region with specific focus on partnership with schools and data integration and communication. Define future strategies and resources needed for school health services primary health care integration. Strengthen health care communications, delivery, and collaboration for children and youth during transitions of care; particularly in settings of foster care, return from incarceration, and long-term hospitalization.

QUALIFICATIONS:

  • Bachelor's Degree required.

  • Graduate degree in Public Health, Nursing, or Health Administration preferred.

  • 8 years minimum relevant experience required

  • 10+ years' experience working with children and families in a community outreach and/or clinical role preferred.

  • Experience working with leaders from various disciplines, including education, health, and human services and parent and family advocates required.

  • Experience with project management, including planning and implementation of standardized workflows/protocols required.

  • Ability to efficiency in prioritizing work, proactively resolving problems, and recommending and implementing continuous quality improvements required. Ability to work independently and meet deadlines required.

  • Excellent skills in leadership, organization, and communication including communications with extramural agencies, officials, primary care physicians and community leaders required.

  • Ability to build multidisciplinary teams and collaborate across clinical disciplines and departments effectively required.

  • Ability to think strategically and operationally at the same time required.

  • Ability to think critically and systemically. Ability to analyze complex problems and formulate evidence-informed plans, solutions and courses of actions required.

  • Knowledge of process and quality improvement methodologies/tools preferred.

  • Ability to analyze, understand, and utilize operational and clinical data preferred.

  • Knowledge of care management, population health, and integrated care principles and best-practices preferred.

  • Knowledge of health behavior change theories preferred.

Pay Range

Pay Range: $95,306 - $142,979 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
