The Lead Foster Care Case Manager collaborates with DCFS to provide appropriate casework services to individuals and families in the Foster Care program including case management and adherence to licensing requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

• Places and supervises children in foster care in compliance with contract regulations.
• Maintains and monitors appropriate placements ensuring time frames are met, documentation is completed and required visits are made.
• Completes all referral forms and other documentation necessary for the effective, efficient, achievement of the permanency goal by time line goals.
• Completes all Caritas Family Solutions paperwork and complies with program expectations as related to the position.

QUALIFICATION REQUIREMENTS: Bachelors Degree in Social Work or related human services field. Two years' experience in providing foster care services or other relevant social work services. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
