Overview

The Lead Care Coordinator provides strength-based, culturally competent Wraparound services, including Wraparound Family Team facilitation, group leadership, and coordination of those providing services to Wraparound families, including the Family Partner and Support Counselor Staff. Additionally, the Lead Care Coordinator can provide coaching, modeling and development opportunities to staff.

ABOUT WRAPAROUND

Seneca offers intensive "Wraparound" services to children and youth at risk of placement in a high-level group home, and to their families. The Wraparound programs are strengths-based, family-driven, flexible, and creative, with the goal of helping the family develop the skills and supports to prevent or reduce the possibility of residential placement of their child. A family-based team addresses the needs of the entire family, not just the at-risk child. This family team consists of family members, other community members who have an important role in the family, and Seneca's care coordinators, family assistants, and family partners. Wraparound serves 34 youth with their contracts - a mix of foster youth, adoptive youth, and youth living with their biological family.

Responsibilities

  • Maintain regular contact with the Family Partners and Support Counselors

  • Guide the Family Team through the phases of Wraparound

  • Maintain a caseload of 3-4 families, or as otherwise determined by the County.

  • Case management and coordination of care

  • Collaborate with county and community partners including but not limited to county social workers, probation officers, outside therapists, psychiatrists, teachers, etc.

  • Coordinate the meeting with the youth's Family Team and develop the individualized clinical assessment and Wrap Action Plan based on the Family Team's strengths and needs. This shall include a comprehensive 24-hour safety plan and shall reflect the best possible fit with the culture, values, and beliefs of the client and family

  • Assist the youth and the youth's Family Team to access needed resources and strength-based mental health services, social services, education services, and other supports and services

  • Provide on-call crisis/emergency services as needed for youth and families who access the program's 24 hour crisis phone line

  • Discuss the provision and quality of activities actually provided with the youth and youth's Family Team and ensure that the activities being provided are responsive to goals and needs identified in the Wrap Action Plan.

  • Maintain all required documentation in a timely manner

  • Attend regularly scheduled staff meetings as per the Supervisor

  • Comply with all requirements of child abuse reporting, Medi-Cal compliance, confidentiality and HIPAA regulations in accordance with standards set by Seneca Family of Agencies

  • Reviews documentation such as mental health notes, Wrap Action Plan, etc. of other Care Coordinators

  • Provides training to staff to help onboard new hires or provide additional support in areas such as time management, note coaching, and facilitation skills

  • Coordinates with Monterey Wraparound and Monterey region's management team to execute program and agency goals

  • Participate in and support management team with hiring processes

  • Perform any other duties as assigned by the Supervisor, Director, or Executive Director of the program.

  • Maintain a flexible schedule that includes early mornings, evenings and occasional weekends

Qualifications

  • Master's Degree in human services, psychology, child and adolescent development, social work, or related field from an accredited college or university

  • BBS Registration required

  • 1 year experience in Care Coordinator role working with children who are part of the child welfare system; experience working with children under age five a plus

  • Strong computer, communication, organizational, and multi-tasking skills

  • Bilingual (Eng/Spanish) preferred

  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements

  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies' insurance policy

  • Proof of up-to-date COVID-19 vaccination or an HR-approved medical/religious exemption

Schedule

  • Full-time

  • Monday - Friday

  • 9:00 AM - 5:00 PM

Benefits

  • Starting salary: $72,461 - $86,461 annually

  • The actual salary is dependent on creditable experience above the minimum qualifications for the role.
