The Social Worker, LCSW is responsible for providing quality and safe delivery of Medical Social Work services that reflect the philosophy and standards of the agency.


  • Provides care and counseling to care of sick and hospice patients and families.

  • Communicates effectively with members of the hospice IDT to promote team centered approach to care, if applicable

  • Attends Hospice IDT meetings if applicable.

  • Performs a comprehensive psychosocial assessment which addresses the relationship between the patient/family's medical condition and psychosocial functioning, environmental issues, problems and strengths of the adult, geriatric, and pediatric patient and his/her support system, implications for the POC and social work goals and treatment plan.

  • Partially or completely meets social work goals related to adult, geriatric, and pediatric patient's medically related psychosocial problems through effective problem improvement or resolution.

  • Provides education and counseling to the adult, geriatric, and pediatric patient and family/support system regarding care needs.

  • Develops, implements and evaluates the plan of care and makes necessary revisions.Addresses all problems in the plan of care.Implements and evaluates orders in a timely manner.

  • Makes recommendations for additional home health care services and community services in a timely manner.Utilizes agency educational material as appropriate.

  • Provides coordination of the plan of care and maintains continuity of client care with other health professionals assigned to the case.Attends client care conferences whenever needed.Initiates client care conferences for the complex and multidisciplinary clients whenever necessary.

  • Documents patient visits accurately and timely, using outcome measures and other social work related documentation tools to facilitate the orders for service, creation of time sheets, and other required patient/employment documents and reports.

  • Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.

  • Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.

  • Performs other duties such as participating in the agency's QI program as required.


  • Master degree in Social Work and/or Board Eligible.

  • Current/Active CT State LCSW Licensure (Licensed Clinical Social Worker)

  • Two (2) year's experience in social work in a health care setting.

  • Maintains current Drivers' License and auto insurance.

  • Possess reliable transportation


  • Demonstrates organizational, written and verbal skills.Demonstrates basic PC skills.

  • Provides medical social work that demonstrates competent social work skills.

  • Demonstrates the ability to perform a comprehensive social work evaluation to establish client level of function, and participates in clients' total plan of care.

  • Demonstrates understanding of social work process, interpersonal relationships and dynamics.Maintains positive professional demeanor.

  • Attends 16 hours of in-services per year.

Understands the mission of MHC by recognizing the elderly we serve as unique persons requiring individual alternatives to maintain full lives in their later years by providing medical, spiritual and emotional care, encompassing quality, dignity, compassion and confidentiality for each individual. Essential Duties and Responsibilities: 1. Visits patients on admission and conducts assessments which are age and culturally relevant and address the patient/residents social, emotional, physical and environmental status. Member of multidisciplinary patient care team. 2. Monitor plans and provides continuing contact with client/family to coordinate ongoing care. Acts as a liaison with nursing/medical staff and community agencies to facilitate problem solving and coordinate services. Acts as an educational resource. 3. Participates in program research activities and develops and implements studies related to client group and social work when assigned. 4. Provides counseling, social support, and assistance in crisis situations and when conflicts arise with service providers. 5. Prepares and maintains required documentation on each resident/patient. 6. Serves as discharge planner on assigned units. Introduces resources designed to meet the needs of the elderly after discharge and make appropriate referrals and arrangements for services. 7. Consults and keeps the Director informed of activities, needs and problems related to caseload. Has involvement in resolution of complaints, abuse allegations, and referrals to adult protective services. 8. Attends meetings as required. Enhances professional growth by attending seminars, workshops and inservices. 9. Understands and follows universal precautions and chemicals that are used in the department. 10. Facilitates groups with residents/families as required per regulation (Resident Council, Family Forum) and develops additional groups PRN. 11. Performs as resident/family advocate and assist team members to ensure realization of resident rights. 12. Educates staff and residents as to bill of rights, including but not limited to grievance and abuse reporting. 13. Maintains on-going communication with managed care insurance carriers for authorizations and notices of non-coverage. 14. Completes Ascend documents for admissions to ACU and SNF and for level of care change notifications. 15. Performs other duties as required. Minimum Qualifications: Education: Bachelor's Degree in Social Work or Master's Degree in Social Work Experience: 1-3 years of experience in a medical or geriatric setting preferred.
