Family Medical Coverage as low as $68.86 per pay period Single Medical Coverage as low as $16.19 per pay period No Mandation 12-Hour Shifts with consistent schedule# Pristine Facility # Job Summary # # The Licensed Practical Nurse provides long term direct care, administers treatments and medications, organizes and distributes daily assignments to direct care staff consistent with staff competency and each individual resident#s comprehensive resident assessment and plan of care.# In absence of the Registered Nurse, supervises direct care staff and makes decisions about resident care needs during shift within scope of clinical competence, consistent with facility policies and procedures.# Routinely exercises independent judgment in completing essential job functions.

Duties # Essential Functions Makes daily rounds on assigned unit at least at beginning and end of shift to observe, examine or interview residents, to verify information from ongoing shift, to monitor regulatory compliance and to determine staff assignments. In absence of the Registered Nurse, develops and distributes resident care assignments to direct care staff.# Assignments should match resident needs with skills of direct care staff, and maintain consistency to the extent practicable.# Adjust assignments according to availability of qualified staff.# Changes assignments if necessary during shift to meet resident needs. Directs daily assignments and evaluates the work of other caregivers, i.e.

CNA#s, CMT#s, Restorative Nursing Assistants, Preceptor Nursing Assistants, etc.# Using independent judgment makes recommendations including, but not limited to the care of residents, discipline, discharge, promotion regarding CNA#s, CMT#s, Restorative Nursing Assistants and Preceptor Nursing Assistants. Education/License/Certification/Other Requirements Minimum requirement: Minimum graduate of an accredited school for practical nursing.# Current and active license as a Licensed Practical Nurse in the state of KY.## Preferred qualifications:

One year experience providing direct care to residents in long-term care setting preferred. Working Environment Constantly works near moving mechanical parts Constantly exposed to blood and/or body fluids and infection Occasionally exposed to toxic or caustic chemicals Occasionally exposed to risk of radiation The noise level in the work environment is usually moderate Physical Demands Occasionally required to maintain in a stationary position (standing or sitting). Frequently required to walk and move about. Frequently required to operate a computer and other equipment.

Frequently required to use fine motor skills to handle and operate instruments/devices. Constantly required to lift and/or move items up to 10 pounds. Frequently required to lift and/or move items up to#50 pounds.
