INFORMATION

Only completed applications received by the specified closing date will be referred to the screening committee for consideration.

To establish a completed application file, the items listed below must be submitted by the specified deadline:

• Completed and signed Pateros School District Employment Application. Application must be completed in its entirety. "See resume," is not acceptable.

• Letter of interest specific to the open position.

• Up-to-date resume.

FOR CERTIFICATED APPLICANTS:

• Two CURRENT (within a year), PROFESSIONAL letters of reference.
• A Washington State Certificate
• A placement file from your college, university, and copies of all college/university transcripts (if selected, the candidate will be required to submit a complete official, unopened, transcripts of college work within 30 days of employment.

FOR CLASSIFIED APPLICANTS:

• One CURRENT (within a year), PROFESSIONAL letter of reference.

• Any additional materials requested on Job Posting/Description.

SUCCESSFUL APPLICANTS WILL BE EMPLOYED ON A CONDITIONAL BASIS PENDING COMPLETION AND CLEARANCE OF THE BACKGROUND/FINGERPRINT CHECK (PURSUANT TO BOARD POLICY), SEXUAL MISCONDUCT RELEASE, AND BOARD APPROVAL.

Applications are retained and considered active for one year following the first submission.

• By submitting this application, this authorizes Pateros School District to conduct a background investigation and authorizes the release of information in connection with my application for employment. This investigation may include such information as criminal or civil convictions, driving records, previous employers and educational institutions, personal references, professional references, and other appropriate sources. I waive my right of access to such information, and without limitation, hereby release Pateros School District and the reference sources from any liability in connection with its release or use. This release includes the sources cited above and specific examples as follows: The Washington State Patrol, information from the Federal Bureau of Investigation of either data on all criminal convictions or certification that no data on criminal convictions is maintained, information received from the OSPI Sexual Misconduct form (sent to former educational employers), information from OSPI, and information from Washington or other State Departments of Social and Health Services and any locality to which they may refer for the release of information pertaining to any findings of child abuse or neglect investigations involving me. Furthermore, I certify under the penalty of perjury under the laws of the State of Washington that I have made true, correct and complete answers and statements on this application in the knowledge that they may be relied upon in considering my application. If the information provided or answer(s) to any question on the application or the Pre-Employment Background Questionnaire change prior to my being hired, I understand that I must immediately notify Pateros School District. I understand that any omission, falsely answered statement made by me on this application, or any supplement to it will be sufficient grounds for failure to employ me or for my discharge should I become employed with Pateros School District. I understand that I may be subject to Washington State Patrol, FBI fingerprint background checks, and clearance on the OSPI Sexual Misconduct Release form as a condition of employment, and/or continued employment. Should I become employed by Pateros School District, and at some future time leave said employment, I further authorize Pateros School District to release information regarding my performance to any potential future employer.

• If an offer of employment is made, Pateros School District will require a criminal record check, including a fingerprint check, through the Washington State Patrol and the Federal Bureau of Investigation to verify your responses to the above inquiries. A signed Sexual Misconduct release form, and subsequent clearance is also required; along with board approval. Successful applicants will be employed on a conditional basis pending completion of the background investigation and sexual misconduct release. By answering yes, I certify (or declare) under penalty of perjury under the laws of the State of Washington that the foregoing is true and correct.
