Juneau Suicide Prevention Training and Outreach Coordinator

The AmeriCorps Member will support our efforts providing Sources of Strength in our high schools and support outreach efforts in the community. Working with our Sources of Strength Coordinator, the AmeriCorps Member will support Sources of Strength programming at Thunder Mountain High School.

This program is designed to create an atmosphere of help-seeking behavior among high school students - seeking help both for themselves and on behalf of friends. The Sources of Strength program meets multiple times each week. The AmeriCorps member will lead group discussions with the youth participants as well as plan larger, school-wide activities with school staff.

In addition to supporting Sources of Strength, the AmeriCorps member will aug-ment our training and outreach efforts to break stigma and educate the community about suicide. This will include tabling community events and handing out brochures, bracelets and other items and planning larger outreach events throughout the year. For example, JSPC holds a Walk of Hope in December where people walk all night at the field house to raise money for suicide prevention.

Member Duties : Major Duties: Plan and perform bi-weekly Sources of Strength events Coordinate with school staff to plan and perform larger, school-wide Sources of Strength events Plan and attend outreach events and activities in the community Distribute resources to partner organizations

Job Overview: This position works closely with the primary medical care team to provide pro-active integrated social services care management that support the biopsychosocial needs of primary care clinic patients. The position coordinates and facilitates smooth transitions between primary care and specialty behavioral health. The position provides supportive counseling, encouragement, coaching, and linkage with essential community services that support each individual's unique healthcare needs. Responsibilities: * Works closely with each primary care provider to assess care needs and local support systems options. * Facilitates and effectively coordinates referrals to community support services. * Establishes and maintains up-to-date knowledge and effective working relationships with local community resources. * Manages referrals to assure linkage with resources. * Develops and maintains a trusting and partnering relationship with assigned patients in order to assist in the success of treatment. * Supports provider decision making and builds on provider-patient interventions. Provides effective support and encouragement of patient needs for lifestyle coaching and/or behavioral health services that are provided at the Juneau Behavioral Health Clinic and Health Promotion as well as outside organizations. * Coordinates and works well with other team members to ensure the efficient and effective flow of patients through the department. * Obtains complete and accurate information in a courteous, respectful and culturally sensitive manner. Attends ELMC staff meetings, training sessions, and planning and problem-solving sessions. Participates in regularly scheduled individual and group supervisory sessions. * Patient notes are documented clearly and timely in order to provide adequate information for other health care providers. Qualifications: Education * BS/BA/BSW in social services, social work, education, human development, mental health or related field. Master's degree in similar field above preferred. Experience * 1 year of experience in general case management, social work, or counseling. Knowledge, Skills & Abilities Knowledge of: * Basic nutrition, physical activity and tobacco cessation recommendations for physical health and disease prevention * Case management principles and application in integrated settings * The use of customer service and recovery skills * Knowledge and awareness of community support resources Skills: * Communication skills to report patient activities and deviations from normal; ability to interact positively and effectively with patients and families. * Use of equipment and software, such as: computers, electronic health record, fax, google drive and phone system * Application of effective and clear oral and written communication * Empathic and supportive listening * Developing interpersonal relationships to work as an effective team member. Ability to: * Maintain strict confidentiality of patient care information. * Maintain documentation in compliance. * Consistently utilize professional communication and conflict resolution skills. * Multi-task and work independently in fast paced, dynamic environment * Maintain compliance with annual competencies * Provide advocacy and outreach * Problem-solve and use conflict resolution skills to develop and maintain good working relations with other staff and departments * Meet patients' needs in a prompt and appropriate manner * Work with individuals of all walks of life, skills and abilities including individuals dealing with various addictions, experiencing homelessness and severe and persistent mental illness * Use the computer system in order to obtain and document patient information. * Effectively coordinate with multi-disciplinary team and SEARHC providers in accomplishing goals/objectives of treatment plan.
