Location: Cape Girardeau, MO

Full Time: Non-Exempt

Work Hours: Monday-Friday: 8:00am

  • 4:30pm

The Job

This position is responsible for the delivery of support staff services and assist in connecting to services for inmates in Cape Girardeau County Jail. These activities include re-entry strategies that involve utilizing local resources for inmates being released back into the community, as well as coordinating communication with transfers to prison. This position functions as a professional staff member of a multi-disciplinary team.

Position funded by a 13-month grant from Missouri Department of Public Safety.

Due to this position being grant funded, in the event the position is no longer required, or due to lost funding, the employee would have the ability to apply and interview for a position at CCC that has similar job qualifications / functions.

Essential Functions

  • Assist Jail Navigator with tracking needs for the program and/or grant; including updating spreadsheets for goals of reducing recidivism rates and use of force.

  • Track, review and follow up with inmates based on Mental Health screening tool.

  • Coordinate transportation for inmates upon release or provide transportation as necessary and appropriate to meet requirements.

  • Maintain positive attitude toward agency and inmates while maintaining appropriate boundaries with inmates and staff members.

  • Provide inmates with appointment times with local mental and physical health providers, as required.

  • Connect inmates to community resources while incarcerated and upon release including referrals to inpatient treatment facilities, specialized courts, or other treatment modalities.

  • Enroll eligible clients in the ERE program for ongoing support and assist with transition to community outreach specialist.

  • Collaborate with the jail medical staff(s) and psych provider(s) to support the inmate with needs, specifically medications, upon arrival and release.

  • Establish a line of communication with the Department of Corrections to provide continuity of care for inmates transferring to prison.

  • Help establish or reestablish services for mental health, during incarceration and after release, including but not limited to admission paperwork, flags, fee agreements, and other documentation and/or by collaborating with the treatment team.

  • Update, copy, track and maintain community resource packets for all released inmates.

  • Assist with referrals to inpatient treatment facilities and/or specialized courts and treatments.

  • Complete benefits applications such as Medicaid and disability.

  • Provide case management type services to adults incarcerated in Cape County Jail.

  • Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.

  • Participate in on call rotation, when necessary and eligible.

Non Essential Functions

  • Maintain confidentiality regarding client and/or personnel matters.

  • Maintain compliance with all policy and procedures.

  • Complete all required training.

  • Work cooperatively with other departments and members of the community.

  • Participate in all required meetings as determined by your supervisor.

Minimum Required Qualifications

  • High School Diploma/GED/HSE

  • Preferred emphasis with secretarial, administrative, and computer courses;or

  • One year experience in similar office setting

  • Travel in own vehicle is required.

  • Must hold or obtain by start date, MO Class E Drivers License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCCs Motor Vehicle Record standards policy and have reliable transportation.

Must be able to pass necessary CPR/FA certification.
