A day in the life of our IVF Nurse Coordinators...

Our Nursing Team is essential to establishing an excellent experience for all our patients. Spring nurses are pillars of support, knowledge and empowerment for our patients, providing hands-on, compassionate care throughout their treatment journey. Our nurses are dependable, compassionate and provide a high level of care. They are detail-oriented and work effectively and efficiently in a fast-paced environment.

Some day-to-day responsibilities include:

  • Establish confidence, trust and a positive rapport with all of our patients
  • Case manage a large patient portfolio
  • Coordinate treatment for patients undergoing Egg Freezing, IUI, IVF and Egg Donation
  • Order time-sensitive medications
  • Facilitate informed consent process
  • Contact patients with test results, updates and treatment outcomes

Our ideal candidate has a Bachelor's Degree in Nursing, a License Registered Nurse in California and has a current CPR Certification.

Department Description The Office of Population Health and Accountable Care (OPHAC) leads UCSF's development of innovative care models and other key strategies to improve the value of care we provide at the population level and facilitate implementation across the health system. We additionally take responsibility for fulfilling our UCSF Health accountable care contracts as well as other clinical partnerships. The Office of Population Health Clinical Programs includes: 1) Complex Care, 2) Care Transitions, 3) Care at Home, 4) Behavioral Health Collaborative Care, 5) Quality Incentive Program, 6) Population Health Outreach Team, 7) Post-Acute SNF Team and 9) Medicare Bundled Payment Programs and 10) COVID-19 operations support. All these program support the platform of the UCSF Health Quality and Analytics, PRIME/QIP Program, the Clinically Integrated Networks and ACO Program and UCSH Health Primary Care and Specialty Care. Job Summary This position is located in Brisbane, CA. This position is a Contract position with duration of one year. The Nurse Coordinator will serve as a resource and liaison for employee, patients and their families. Assists with navigating all aspects of the healthcare system related to COVID. Uses knowledge of the unit or clinic to resolve issues and identify ways to increase patient satisfaction. Collaborates with department managers to meet patient expectations, achieve quality outcomes and build a patient focused culture. The Nurse Coordinator will be responsible for having a broad understanding of the services provided by the UCSF OPHAC Clinical Program/COVID Hotline and Triage and duties may include: * Providing services to employees and patients as directed by the COVID Hotline and Triage protocols under medical direction guidance * Assisting in triaging and communicating concerns and/or change of condition in the patient to the appropriate medical supervisor and other staff member * Assisting health care team in setting up telehealth appointemnts, in-person visits and continuing management of COVID positve patients * Collaborate with the clinical teams as needed to support the ongoing care of empanelled patients This Nurse Coordinator works as part of a dynamic care management team and will be responsible for monitoring telephone calls and gathering all necessary pertinent information regarding medical problems, symptoms and beneficiary questions using the nursing process. Ensures COVID employees and patient calls are appropriately and safey resolved, or being routed to the most appropriate resources through assessment, planning, implementation and evaluation. Ensures all necessary calls and messages are handled in an expedient manner. Required Qualifications * Three years recent experience as a registered nurse caring for ill adults and older adults * Bachelor's of Science in Nursing from an accredited school of nursing * Demonstrates communication and problem-solving skills necessary in patinet/family and healthcare team interactions * Demonstrates knowledge of the principals of growth and development over the life-span and incorpates this knowledge into plans of care and/or goals of care * Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations; knowledge of Medical Terminology; strong knowledge of data collection, compilation, and analytical techniques * Strong skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance; in-depth knowledge of the organization and how to get issues resolved * Skills to work independently in a fast-paced, demanding environment with minimal supervision * Ability to manage and oversee multiple tasks simultaneously, including high daily call volume * Knowledge of medical terminology * Excellent written and verbal communication skills * The flexibility to orient and work at all UCSF Health locations Preferred Qualifications * Certification in Geriatrics, Palliative Care or Case Management in Nursing * Experience developing work flow procedures and implementing systems to improve operations * Experience in inpatient, outpatient or homecare settings * Experience with telephone triage * Proficiency with EPIC APeX applications Licensure/Certification * Active Registered Nurse licensure in the State of California * Active American Heart Association CPR Certification Living Pride Standards Service Excellence * Demonstrates service excellence by following the Everyday PRIDE Guide with the UCSF Medical Center standards and expectations for communication and behavior. Removes and reports broken equipment and furniture. * Picks up and disposes of any litter found throughout entire facility. * Posts flyers and posters in designated areas only; does not post on walls, doors or windows. * Knows where the Environment of Care Manual is kept in department; corrects or reports unsafe conditions to the appropriate departments. * Protects the physical environment and equipment from damage and theft.
