Position Summary:Interventional Nurse RN Navigator - Imaging

The Interventional Radiology Navigator is responsible to work collaboratively with the patient navigator, the patients, their families and healthcare provides to organize and coordinate all aspects of the patient's treatment.

The Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. S/he facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The incumbent guides patients through the health care system by assisting with access issues, developing relationships with providers, and tracking interventions and outcomes. Serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services

SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS:

  • Streamline care coordination and paperwork of patients by scheduling appointments and assisting with exam and procedure preparation.

  • Conduct outreach to referrers, providers and other medical professionals.

  • Function as a member of the multidisciplinary team as an advocate and educator for patients from point of entry, through diagnostic studies, diagnosis and treatment plan.

  • Build relationships with patients and physicians, to coordinate a plan of care including appointments, transportation, education, provision and / or enablement of support services and representation within the multidisciplinary care environment.

  • Serve as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, referrers, support network members e.g. social workers, and the wider healthcare community. This role may include conducting internal and external outreach and marketing.

  • Collaborates with the radiologist and primary care physician in provision of comprehensive and multidisciplinary care for the patient and family.

  • Manage efficient flow of patients including screening, preparation for exam, providing general nursing care, and arranging or testing and admissions

  • Instruct patients and families regarding medications and treatment instructions

  • Administer medications in accordance with physician orders

Qualifications

Education:

  • Bachelor of Science in Nursing (BSN) Degree preferred

  • Tuition Reimbursement Program (up to $5,250/calendar year)

  • HCA will pay for certification renewals once employed

Experience/Qualifications:

  • 3-7 years of related experience
  • BLS (American Heart Association Health Care Provider or American Red Cross)
