The Patient Care Coordinator will act as a liaison to patients and families referred to the Nurse Clinical Care Managers to arrange appropriate KKI services related to the patient's special needs. This population includes inpatient, outpatient, and International patients. This position requires a high degree of customer service, strong relationship building skills, and cultural sensitivity to manage the needs of clinical staff, patients, and their families. The Patient Care Coordinator will function as a team member with the Nurse Clinical Care Managers.
ResponsibilitiesResponsible for intake coordination for referrals specified by the Nurse Clinical Care Manager. Coordinates and disseminates patient information for review by physicians and Clinical Care Managers.Obtains and documents patient clinical, demographic, and insurance information in designated KKI forms and databases.Researches and documents previous appointments and/or requests for services by internal and/or external referral sources.Communicates with appropriate staff for approval or appropriateness of referral. Follows up with staff and completes all documentation. Finalizes the intake by communicating disposition to the family and/or referral source.Verifies patient is correctly scheduled and insurance clearance is obtained prior to the appointment date. May enter information in designated KKI systems such as AS400/IAMS. Will have access to POE and EPIC.Schedules appointments with the Care Center and/or Clinic Coordinators based on Clinic Policy. Notifies Clinical Care Associates of any critical issues/information regarding appointments, insurance, referrals, Letters of Guarantee, etc., as needed.Serves as a resource and facilitator to patients and families to explain appointment locations, appointment schedule, and services provided at the Institute. Ensures that the patient's transition between services goes smoothly, including copying and distributing medical records prior to the visit, and participates in follow-up scheduling as directed.Creates and maintains databases for designated inpatient and outpatient referrals, including appointments, surgery dates, and follow-up appointments as requested.Maintains an ongoing relationship with the Kennedy Krieger Referral Liaison and attends outreach meetings as needed.Produces regular reports of patients served for review by the Director of Care Management Operations and KKI Administration.Qualifications
High School Diploma or GED required.
Bachelors' Degree in human services or health care administration field preferred.
At least three (3) years' experience coordinating patient services in a medical setting with children and families, preferably those with special needs, required.
Proficiency with Microsoft Office Suite and other business-related software required.
Knowledge of health insurance and medical terminology.
Knowledge of KKI services and specialties preferred.
