Overview
Unity Center for Behavioral Health is a collaboration between area hospitals and provides 24-hour mental and behavioral health services in Portland, Oregon. As a nurse practitioner at Unity, you will be employed by Legacy Health. Their mission statement is Good health for our people, our patients, our communities, and our world.
Unity's internal medicine consult service works in collaboration with the primary psychiatric team to diagnose and manage co-occurring medical illness within hospitalized psychiatric patients. (Note: this is a medical nurse practitioner role, not a psychiatric nurse practitioner role).
Our team is excited to be expanding; we are urgently hiring part-time nurse practitioners to join our close-knit service.
This is a unique position that blends aspects of hospitalist, primary care, and urgent care provider roles. We do not have any admitting or discharge responsibilities. Our work is completed during our scheduled shift, there is no inbox to address outside of our scheduled hours.
Responsibilities:
Advising the primary psychiatric team regarding care for chronic and episodic medical health concerns and formulating related plans of care
Triaging medical consults and autonomously rounding on patients across the adult, adolescent*, emergency psychiatric, and emergency substance use disorder units
Performing routine medical history and physical exams
Chart review, physical assessment, diagnosis and management of internal medicine conditions such as diabetes, heart failure, hypertension, chronic kidney disease, basic integumentary conditions, falls, headaches, musculoskeletal pain, altered mental status with concern for primary medical etiology
Responding to consult requests and questions via office phone, EPIC chat, and pager
Ordering and interpreting diagnostic tests to identify and assess patients' health care needs
Ordering medications for physical ailments
Documentation of pertinent health information using EPIC EMR
Determination of level of care warranted according to the Unity Medical Exclusion Criteria
Leading BLS-level response to medical codes together with psychiatric staff
Engaging in multidisciplinary complex medical case discussions
Supporting care coordination for medical transfers to partner hospitals
Seeking clinical decision-making support, when required, from the hospitalist physician team at Legacy Emanuel Medical Center, and subspecialists across the Legacy and OHSU networks
Additional information:
Seeking provider for 0.6 FTE (average 24 hours/week over 13-week quarter)
Flexible schedule: 9-hour day shifts (0800-1700), rotating weekends and holidays, opportunity for stretches of several days off without use of vacation days (APL)
Variable patient load based on number of consults; average 7-10 patients a shift, including a mix of new consults, follow-up visits, and chart checks
Collaborative role with on-shift clinicians dividing the daily patient load and partnering on challenging cases as needed
Opportunity to spend extended time with patients (>1 hour common) and see patients several days if not weeks in a row
BLS facility, with a matching level of acuity (e.g., no tubes/lines/drains, imaging, advanced procedures)
Numerous safety precautions in place to allow for care of a complex patient population, including secured units and a robust 24/7 security presence on-site
HRSA designated site with the potential for related loan repayment
Work life balance with no panel or patient inbox responsibilities; ability to feel totally free from work on days off
LICENSURE/CERTIFICATION REQUIREMENTS:
National Certification as a Nurse Practitioner (NP) in applicable specialty (e.g., FNP, ANP, AGACNP)
Current Oregon State license as a Nurse Practitioner (APRN) with Prescriptive Privileges (NP-PP)
Current Oregon State RN license
Basic Life Support (BLS) certification
Must be DEA eligible upon hire
*Certification to treat adolescent patients (ages 9-17) in addition to adult patients is preferred, but not required
*Experience with medical management of patients in roles such as primary care, internal medicine, hospitalist, urgent care, emergency medicine, or intensive care, is preferred but not required.

Pay Range
