Re-Posting dates for internal transfers: May 12, 2023, through May 14, 2023 BASIC FUNCTION Pharmacy Interns perform all drug procurement, preparation, and dispensing tasks similar to our AACH Certified Pharmacy Technicians, plus perform pharmaceutical care and MTM tasks (e.g. #Patient home medication reviews and reconciliation, DUEs, drug formulary reviews, etc.), all under the direct supervision of an AACH pharmacist. # Pharmacy Intern positions may be structured as casual part-time, part-time permanent, part-time temporary, or full-time temporary, depending on specific need. # JOB DUTIES # A. Provides Pharmaceutical Care (See HFAP 25.00.00) under the direction of a pharmacist # Procures and distributes medications under the supervision of a pharmacist, pharmacy resident, or pharmacy intern and by prearranged work assignment, according to established policies, procedures, and protocols. Maintains adequate stock in all automated dispensing machines. Refills machines in a timely manner according to established protocols. Inventories and replenishes medications in emergency crash carts, anesthesia kits, surgery trays, and several other forms of assembled kits. Assures all items are pharmacist-checked once completed. Prepares compounded sterile products (CSPs) (e.g. IV piggybacks and large volume parenterals, parenteral nutrition, patient-controlled analgesia solutions, epidural infusions, etc.) accurately, following policies and procedures compliant with USP #797# and USP #800# Checks in, distributes, and stocks controlled substances and dangerous drugs within the facility, and maintains all necessary recordkeeping as required by law and department policies Successfully completes a Media Fill Challenge, annually or as required, in accordance with USP #797# requirements. Prepares and fills Colleague, discharge, hospice, clinic, and outpatient prescriptions in a timely manner using the QS/1 computer system. Provides patients with their filled prescriptions according to standard procedures, and completes any required payment transactions (e.g. processed credit card payment, fills out sales paperwork, etc.).## Checks in and signs out controlled substances from secure electronic cabinetry per policy and procedure. Maintains adequate stock of in-date medications and supplies, appropriately stored and secured throughout the facility. Troubleshoots operational problems with our automated dispensing systems and serves as the first line of repair. Assists and accompanies Omnicell company service staff whenever on-site performing repairs, preventive maintenance, or upgrades. Completes patient home medication reviews and reconciliations for patients being admitted and for other patients as may be requested or assigned. The home medication review is performed in a structured manner, gathering information from the patient, family or caregivers, retail and mail-order pharmacies, and other healthcare facilities to assemble as complete, detailed, and accurate current medication list as is reasonably possible. In doing so, the pharmacy intern assists the pharmacist and other members of the healthcare team by providing key components of Pharmaceutical Care: # Determines the presence of medication-therapy problems, both potential and actual Summarizes the patient#s medication-related health care needs. Identifies and specifies pharmacotherapeutic goals for patients. Monitors the effect of the pharmacotherapeutic regimen. Redesigns the regimen and monitoring plan as indicated. # Provides education to patients and family members/caregivers regarding medications, how to take or administer them, their effects and side-effects, and how they may affect or improve the patient#s disease or medical condition. # Provides education to patients on selected diseases or disease processes (e.g. thromboembolic disease and anticoagulation, heart failure, diabetes, etc.) # Maintains and demonstrates competency. Maintains Ohio licensure as a Pharmacy Intern at all times. # Seeks out and completes qualified education opportunities to enhance skills related to his/her personal practice environment. # Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy. Compounding Aseptic Isolators (CAIs) - for the preparation of Compounded Sterile Products (CSPs) # EDUCATION, TRAINING # EXPERIENCE: # The applicant must be currently enrolled in an ACPE-accredited Doctor of Pharmacy (PharmD) degree program. # Applicants are expected to maintain a high level of academic success while enrolled within the Doctor of Pharmacy (PharmD) degree program. The applicant must be a US citizen or hold a valid visa as a foreign student.
