To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 04/08/2021. Time-In-Grade Requirement:Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service.An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. You may qualify based on your experience and/or education as described below: GS-07 Specialized Experience:Do you possess at least one (1) full year of specialized experience, which equips you with the particular knowledge, skills, and abilities to perform successfully the duties of the Intermediate Care Technician (ICT) PACT, GS-07 position, and that is typically in or related to the work of the position to be filled. To be creditable, this specialized experience must have been equivalent to the grade 05 level in Federal service to qualify at the grade 07.Examples of specialized experience include:1) The incumbent assists in behavioral/ psychiatric situations in an outpatient setting.2) Performs 12-lead electrocardiograms3) Performs observation-based screening and collection of patient information in collaboration with licensed personnel.4) Assists licensed personnel with the delivery of care to patients with complex and specialized care needs5) The incumbent operates specialized medical equipment to obtain diagnostic and therapeutic outcomes. Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week.Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements:The work involves extended periods of bending, standing and walking. Generally, the work requires recurring physical exertion with assisting patients. On a regular and recurring basis, employee is exposed to infection, exposure to communicable diseases and contagious diseases. Learn more about this agency Responsibilities The Care in The Community (CITC) Case Manager will identify patient care needs, establish priorities within an expanding patient population in order to effectively and efficiently manage the comprehensive needs of this population. The CITC Case Manager will coordinate services with other team members in order to promote positive patient outcomes. This nurse will closely manage specific populations requiring outside services, expediting care across a continuum or those patient groups where timeliness of services is critical for a positive outcome. These special populations include, but are not limited to, Vascular, Surgery, Pulmonary, Oncology, Cardiology, Endocrinology, and Oncology Radiation. Coordination of Care is also provided for outside services referrals to General Surgery, ENT, Plastics, Urology, Sleep Studies, Gastroenterology, Rheumatology, Orthopedics, Neurosurgery, and General Medicine referrals. The Case Manager is the coordinator of the process, reviewing outside services consults for accuracy and appropriateness, assisting with the coordination of services between referring and treating facilities, and problem-solving with patients and supportive staff as necessary. This Case Manager is responsible to perform community visits (when necessary) in support of patient needs and transition back to Lebanon VA Medical Center or to their residence. The Case Manager performs business case analysis comparing actual care to Diagnosis-Related Group codes to accurately predict and ensure billing is accurately projected and overbilling does not occur. Coordinates admissions and assists with bed control functions to transition Veteran inpatient care back to Lebanon at the earliest possible juncture. Serves as admission coordinator, working closely with LVAMC and Civilian hospitals in coordination of inpatient admissions. Performs business case analysis for purchased care to ensure the highest level of care at the most economical costs. Works closely with fee resources and civilian hospitals to ensure timely and accurate invoicing and payment for services. The Case Manager assist with interfacility VA transfers as well as the Traveling Veteran. The Case Manager provides effective communication and collaboration with members of the interdisciplinary team, the Veteran, and the Veteran's family members to ensure the Veteran receives timely and appropriate care to meet the Veteran's needs. Work Schedule: * 8:30 a.m. - 4:30 p.m. Monday through Friday * Part-time telework available upon approval / subject to change Financial Disclosure Report: Not required. Physical Requirements: The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. Preferred Experience: 5 years Acute Care experience preferred.
