The full performance, Intermediate Care Technician (ICT) CORE/ Generalist is assigned to the Medicine Specialty Clinics for the Department of Veterans Affairs VISN 7 Clinical Resource Hub. This position serves as an advance technician for outpatient care settings such as; primary care, mental health, and specialty clinics. As an ICT Generalist the incumbent may provide care in a medical center, Community-Based Outpatient Clinic (CBOC), mobile medical unit or may have virtual/remote assignments.

Duties

The incumbent performs ongoing patient care duties in an outpatient clinical setting. ICT's may perform tasks in accordance with local policies after documented completion of training and demonstration of appropriate skills and competency.

These tasks are not all inclusive and require the demonstration of appropriate training and documented competence prior to their performance. The incumbent may assist with or perform other related approved technical health care procedures under the direction and supervision of the licensed personnel as competency is demonstrated and documented. Duties include but not limited to:

  • Patient Screening: Performing observation-based screening and collecting patient information in collaboration with licensed personnel.

    This may involve clinical reminders, obtaining patient health history for chief complaints, medication history, and information about health-related or high-risk conditions that may impact the patient's health status. The incumbent will complete approved templates and tools, such as the outpatient intake form, normal results notes and follow up treatment protocols established by the agency.

  • Clinical Tasks: The incumbent assists licensed staff with the delivery of care to patients with complex, specialized and potentially life-threatening situations.

    This may include setting up equipment and passing instruments and providing treatment during visits. Collaborates with the health care team to help identify the nature and extent of a patient's condition or seriousness of the injuries or illness of the patient's care requirements. Observes patient status and keeps licensed clinical staff informed of any change in condition.

    Utilizes complex technical equipment for clinical video or store-and-forward examinations to assist with virtual physical and/or mental health care (i.e. total exam camera, dermatoscope, digital stethoscope). This includes troubleshooting and fixing telehealth equipment as initial point of contact if possible, as well as training new telehealth providers and other telehealth staff to VA telehealth standards. Has clear understanding when need for technical support and assistance with equipment.

  • Clinical responsibilities include but are not limited to: Vital signs.

    Examples: vital signs, 12-lead electrocardiogram (EKG). Point-of-care (POC). Examples: Abbott hand-held blood analyzer (I-STAT), glucometer. Equipment and surgical instrumentation set up.

    Examples: Biopsy (Excisional, Shave, Punch) Observations and interventions. Examples: cryotherapy, cautery, biopsy, laceration repair, incision and drainage. Wound care.

    Examples: bandaging, application of dressings, wound measurements, photo-documentation of wounds. Specimen collection and Preparation. Examples: tissue collection, punch biopsy and wound cultures.

    Assisting with patient exams and screening, utilizing technology such as telehealth peripherals. Examples: primary care physical exams (heart, eye, ear, nose, throat (EENT); gait exams, skin exams, tele-dermatology and tele-podiatry.

Qualifications:

The Selective Placement Factor for this position is: Knowledge of the major body systems, i.e., skeletal, muscular, nervous, respiratory, circulatory, digestive and genitourinary, to perform patient examination and to provide prompt and efficient patient care and appropriate patient handling; working knowledge of topographic anatomy, e.g., right/left, surface, proximal/distal, superior, inferior, to accurately transmit location information about the body; knowledge of medications (their uses, doses, side effects, adverse effects and toxicity signs) sufficient to properly administer drugs even when directed to do so by the licensed staff; knowledge of major psychiatric conditions, depressive, anxiety, bipolar and substance use disorders, and schizophrenia.

You may qualify based on your experience and/or education as described below:

Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-06 in the normal line of progression for the occupation in the organization.

Examples of specialized experience would typically include, but are not limited to: Monitoring vital signs, administering medication; performing observation-based screening and collecting patient information in collaboration with licensed personnel; assists in behavioral/psychiatric situations in an outpatient setting; assists outpatient licensed staff with the delivery of care to patients with complex, specialized, and potentially life-threatening situations; preparation of lab specimens and transfer to the lab for processing; Treatments and interventions which include Cryotherapy, incision and drainage, ambulation, and Foley coude' catheter insertion.

Education: one full year of graduate education with the subject area of medical health or related fields. This education must have been obtained in an accredited college or university. (A transcript must be submitted with your application if you are basing all or part of your qualifications on education).

Preferred Experience: Former Military Medic experience is highly desired (Ex: Armed Forces Corpsman or Medic), minimum of 1 year of specialized clinical experience.

You will receive credit for all qualifying experience, including volunteer experience.

Note: A full year of work is considered to be 35-40 hours of work per week.

Part-time experience will be credited on the basis of time actually spent in appropriate activities. Physical Requirements: The work involves extended periods of bending, standing and walking. Generally, the work requires recurring physical exertion with assisting patients.
