Reporting to the Director of Social Services, the IDN (Integrated Delivery Network) Program Manager is responsible for the oversight of the implementation of the Scope of Work in MCHCs contract with Network4Health (the name of our regional IDN). This position will be critical in the implementation of new programs and workflows, communication with our contacts at the IDN, management of the budget and related expenses, working with management and staff on workflows to support the new integrated care model, and internal communication efforts to both staff and eventually patients about changes in our model of care.
QUALIFICATIONS: Education/Training:
Bachelor Degree in Social Work or equivalent qualification Experience: Two years experience in healthcare field.
Must demonstrate knowledge of community resources and the ability to network.
Experience with low-income and multi-cultural populations necessary.

To perform this job successfully, an individual should have knowledge of Word Processing software.
Skills: Ability to work independently.
Verifiable good driving record and reliable transportation Background check RESPONSIBILITIES: Responsible for working closely with the Director of Behavioral Health and the Director of Social Services in the development of the infrastructure to support the delivery of integrated care, and working closely with their direct reports that supervise CHW and Case Management staff.
Responsible for working with other managers to develop the case management and CHW roles and ensuring these roles are well-integrated within multi-disciplinary teams at all MCHC sites.
Works closely with the CMO and COO to assure integration of care plans are working well within the primary care paradigm.
Responsible for facilitation of the IDN Task Force, to include preparation of agenda and handouts, meeting facilitation, minutes distribution, and follow up on appropriate items.
Works closely with CHAN in the implementation of the technical part of the SOW (Scope of Work) for the IDN strategies.
Represents MCHC at various IDN activities both locally (Network4Health) and state wide as appropriate.
Works collaboratively with external agencies and hospitals in the facilitation of care coordination agreements and referral systems.
Responsible for the regular completion of IDN reports and assessments as agreed to in our contract with Network4Health and sharing those reports with the senior leadership and the Task Force.
Responsible for the overall budget for the IDN SOW and regularly updating senior leadership and managers involved in the Task Force about our progress on the budget.
Will work with other departments and their managers to communicate and educate staff about various components of the model being developed.
Attends MCHC administrative agency meetings and committee meetings as assigned.
Attends seminars and educational programs necessary to gain knowledge and training in selected areas.
May be asked to represent MCHC on selected community coalitions and at selected community meetings.
Performs administrative duties as assigned and appropriate to the functioning of the department.
