Intensive Case Manager BA - CBFS

Eliot's Adult Mental Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that promote consumer rights, responsibilities, rehabilitation, and recovery. We provide rehabilitative services that enable people to increase functioning in the living, working, and social environments of their choice.

Join a committed team of professionals in providing counseling and recovery services to adults with mental illness at our Concord CBFS outreach Program.

Responsibilities:

Provide case management services to individuals living independently in the community.

Develop IAP's (Individualized Action Plans) with each consumer on case load in collaboration with supervisor.

Collaborate with each consumer in developing interventions related to the IAP that are clinically specific to behavioral issues.

Develop intervention-based groups to address the needs of the population.

Schedule: Monday-Friday 9a-5p

Pay rate: $17.52 hr

Requirements

Requires a BA/BS in related field with at least two years of experience working with people with severe and persistent mental illnesses or a Masters' Degree in a related field and/or a clinical license. Must have at least one year of experience working with a designated specialized population: dual diagnosis, transitional-age-youth, behavioral, forensically involved, medically compromised, experience with family systems, brain injury or those requiring dialectal behavioral therapy. Ability to develop groups and individualized interventions for the assigned specialty population.

Responsibilities Include: * Manages department operations and supervises frontline supervisors as well as technical staff that are responsible for all incoming customer inquiries and support tickets * Assigns and coordinates work assignments and resolution of critical system problems. * Demonstrate effective team management skills in actions, communications, and management practices, while maintaining a strong customer focused approach * Gain a comprehensive view of the team's strengths and abilities with an emphasis on staff development to enhance individual results thereby strengthening the entire team * Create and maintain team performance metrics * Create, track, analyze and report a wide range of metrics on a regular basis in addition to ad hoc reporting. * Serve as escalation point for elevated and customer critical issues * Meet with customers as needed either in person or via phone to review case management, SLA performance, and elevated cases * Manager must have the ability to work effectively across departments, as operations impacts multiple functional areas in the company. * Must have strong presentation skills and be able to represent the organization during customer meetings and user trainings as needed. * Analyze system data and case trends in an effort to get ahead of and prevent system issues * Develop and maintain standard operating procedures and policies for the department Qualifications * BS or BA in Bus Amin, Communications or Computer Science or 5 years of proven work experience * 2 – 4 years as a lead agent or lead coordinator in a call center environment * Proven leadership experience in a call center or technical environment with an ability to delegate, motivate and train large complex teams * Considerable decision making capability, sensitivity to accuracy and timeliness, ability to train and evaluate support staff * Strong client facing and communication skills * Experience with Salesforce CRM tool preferred * Strong analytical skills and able to organize and interpret data from various sources * Understanding of hospital procedure area workflows preferred * Basic understanding of Linux and SQL preferred * Familiarity with network base monitoring tools * Ability to effectively manage multiple tasks, prioritize as necessary so that all clients receive the highest level of customer service.
