Intensive Adult Case Manager - Bangor Area

Description

OnCourse Maine, a program of Androscoggin Home Healthcare + Hospice, has an immediate opening for an Intensive Adult Case Manager. Part and full-time options are available. This position provides support to Veterans in their respective homes who have severe and persistent mental illness(es). The Intensive Adult Case Manager assesses Veterans needs, develops, implements and reviews their treatment plans, and coordinates with other community resources in meeting/achieving Veterans service needs.

Duties and responsibilities include:

  • Serving as the primary support person and educator to the consumers identified supports and coordinate with the VA regarding available resources located in the community that are needed during the course of treatment.
  • Assisting Veterans and families in developing goals and areas of need, and assisting in social development, assessing them quarterly.
  • Serving as the primary point of contact when the Veteran is in crisis.
  • Maintaining case records and completing appropriate documentation.

Requirements

  • Candidates must have a MHRT/C and a Bachelors or higher degree in Social Work, Nursing, or another health or human services field that promotes the physical, psychosocial, and or vocational well-being of the persons being served. A minimum of two years experience in direct service is preferred. Prior experience in a behavioral health or decentralized environment is a plus. All applicants must have a valid Drivers License, current Auto Liability Insurance, and reliable transportation.
