Integrated Support Care Coordinator

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County.

We are seeking a full-time Integrated Support Care Coordinator to support our Brentwood, NY site. SUMMARY OF POSITION: Provides integrated care coordination support to care teams and patients across the patient care continuum.

Facilitates the coordination of completing all reports and case reviews in a timely manner. Assist with team building and increasing staff capacity to address patient social determinants of health through coordination of team meetings and training, facilitate chart reviews and works alongside leadership to complete quality improvement projects. Performs other duties as assigned on an as needed basis.

ESSENTIAL FUNCTIONS: Participates in team meetings, advising on project successes, challenges, and strategy to support completion of project deliverables. Serves as a backup support to care managers.

Conducts chart audits and review. Assists with distribution and completion of intakes. Empowers, educates, and links participants to HIV, HCV, STI testing/treatment, PrEP, substance use, and other primary care services.

Collaborates in the identification and coordination of team training. Attends related training to support organizational initiatives. Fulfills related administrative duties as needed.

Develops and coordinates harm reduction outreach strategies appropriate to engage patients. Provide guidance and support to staff. EDUCATION/EXPERIENCE:

Minimum Requirement: Bachelor's Degree Preferred Education: Master's Degree Credentialed Alcohol and Substance Abuse Counseling Certificate Program (CASAC) 2-4 years' experience, preferred.

Job Type: Full-time Salary Range: $23.00 - $28.75 per hour
