We are seeking a full time Primary Care Psychologist to join our practices in the Denver, CO area. The Primary Care Psychologist will serve as a key member of the integrated primary care practice, playing a critical role in serving the behavioral health needs of our patients. The Primary Care Psychologist will work closely with the Medical Director, Clinical Team Manager, and Director of Behavioral Health.

Position Highlights:

  • Co-manage the assessment and treatment of psychiatric concerns, psychosocial issues, and health behavior change through consultation and brief, focused assessment and treatment.

  • Provide education and consultation to health coaches and clinicians on the basic principles of brief behavioral interventions and reinforcing their use in medical visits.

  • Evaluate crisis situations and employ appropriate interventions.

  • Facilitate group-based visits and workshops to holistically address medical and behavioral health needs.

  • Improve population health outcomes by ensuring all patients and primary care team members have access to behavioral health expertise.

  • Facilitating and overseeing referrals to specialty mental health services and supporting a smooth transition from specialty mental health services to primary care.

  • Collaborate with other Behavioral Health Specialist's across Iora Health to improve behavioral health skills company wide.

Qualifications:

  • Previous experience in an integrated care setting highly desired.

  • Must be currently licensed, or license eligible, to practice independently in Colorado state. (PSY, LCSW, LMHC, LCPC, MFT licensure accepted).

  • Preferred: Highest degree in academic concentration (PhD, PsyD or equivalent).

  • Required: Masters degree in psychology, counseling, social work, marriage and family therapy or related field.

  • Has excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.

  • Is highly skilled in cognitive behavioral therapy and/or acceptance and commitment therapy and motivational interviewing.

  • Has exceptional capacity to multitask, is flexible, and is comfortable with ambiguity.

  • Has strong communication and teaching skills.

  • Spanish fluency highly desired

Summary: Provides triage, assessment, determination of appropriateness for admission or referral, treatment, education, utilization, provision of resource information and disposition planning to patients and families, in particular those patients with mental health, psychiatric and substance abuse diagnoses. Work Schedule: Full Time Days Responsibilities: Develops individualized treatment plans, provides care/case management and directs appropriate disposition. Supports all activities with appropriate documentation. Provides group and individual counseling services using accepted psychotherapeutic models and interventions, including integrative healthcare options. Assesses, collaborates and coordinates patient's care and discharge planning needs and readiness for discharge with colleagues in Care Management and Medical Social Work and other members of the care team. Coordinates psychosocial care as needed with third parties and community agencies. Initiates psychiatric holds and Emergency Commitments as appropriate. Responds to referrals, incidents, or problems, provides coverage as assigned, assists staff with cases as needed. May verify benefits and supports financial counseling. Orients patients and their families to the requirements, policies, and goals of the proposed treatment program. May provide clinical supervision for LCSW, LCSW candidates, LSW; as well as MSW, MFT, PC, and other students and support staff in relevant disciplines as assigned. Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action. Requirements: * Minimum Required Education: Master's degree in Social Work.
