Integrated Behavioral Health Clinician In Primary Care Setting / Full-Time / Days

Overview Chestnut Family Health Center, a division of Chestnut Health Systems, is seeking a full-time Integrated Behavioral Health Clinician for our Bloomington, IL primary care facility. This position is eligible for a sign-on bonus Chestnut Health Systems is a leading provider of behavioral and primary health care services.

Responsibilities Provide services identified in individuals’ treatment plan that are consumer driven and integrates evidence-based practice concepts, including motivational interviewing and therapeutic alliance. Actively participate in Quality Improvement/Quality Assurance related activities and program development activities.

Promote the recovery model of treatment including hope, respect, empowerment, health/wellness and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery. Provide for advocacy, and referral and linkage to recommended community resources. Assist patients to identify individuals’ strengths, resources, preferences, choices and natural supports.

Actively participate as a team member in multidisciplinary team meetings and patient case staffings. Provide individual, group, family and collateral interventions. Provide appropriate crisis intervention.

Qualifications Must have a master’s degree in a human services field and qualify as a Licensed Practitioner of the Healing Arts (LPHA) in the state of Illinois. Illinois licensure of LMFT, LCSW or LCPC required. Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR). Must have a valid driver’s license, private automobile insurance and be insurable (when applicable). Satisfactory completion of Chestnut’s background checking process.
