POSITION SUMMARY: The Intake RN is an experienced registered nurse who is accountable for providing the initial point of contact to patients coming into UW Health system.

The Intake RN is well-versed in the patient population and understanding the appropriate resources and care teams to help meet the patient#s needs.

The RN reviews the patient#s medical history and referral to determine the most appropriate care team for the patient and then works in collaboration with a team member to schedule the patient in a timely manner. The Intake RN also connects the patient with the appropriate RN Navigator based on the diagnosis or symptom.

The Intake RN will demonstrate clinical assessment expertise, triage expertise and outstanding customer service throughout the interaction for both external and internal users. The Intake RN must communicate effectively with faculty, staff, and external contacts using tact, discretion, and diplomacy.# This person must possess well developed problem solving and team development techniques.# Key responsibilities include direct patient care, participation in staff development and education, and quality improvement initiatives. The Intake RN delegates and supervises care in accordance with the and UWNIL Health Nursing Professional Practice Model.

The Intake RN participates in continuous quality improvement initiatives and assumes responsibility for their professional development. EDUCATION/TRAINING: Minimum: Bachelor#s Degree in Nursing Preferred:

Master#s in Nursing, Social Work, Public Health, Hospital Administration or related field LICENSURE/CERTIFICATION: Minimum: RN Licensure in State of Illinois, CPR/BLS Preferred: N/A EXPERIENCE:

Minimum:

Six (6) months of relevant clinical experience Preferred: N/A REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES: Experience in program development and evaluation Excellent organizational and time management skills, with attention to detail.

Knowledge and experience documenting patient care in an electronic medical record. Excellent verbal, written and presentation skills. Ability to make knowledgeable clinical assessments and judgments.

Ability to educate staff and families. Ability to provide counseling and emotional support with compassion and respect for the individual/family Ability to provide support to the members of the care team (teamwork) Must possess initiative and ability to work independently. Ability to maintain confidentiality of medical records.

Ability to function with a high degree of independence.
