Description:

Responsible for all Regional Rehabilitation Intake functions including establishing and maintaining

positive relationships with customers and referral sources, responding to customer requests and

concerns, confirming appropriateness for admission into Loyola's Post-Acute Rehab Services. The

individual conveys a professional and positive image of Loyola Medicine during interactions with

patients, families, physicians, staff members, and internal departments. The individual provides service

reflective of the mission, vision, and values of Loyola Medicine.

concerns, confirming appropriateness for admission into Loyola's Post-Acute Rehab Services. The

individual conveys a professional and positive image of Loyola Medicine during interactions with

patients, families, physicians, staff members, and internal departments. The individual provides service reflective of the mission, vision, and values of Loyola Medicine.

40% Yes Patient care- Reviews clinical status of potential post-acute rehab patients to ensure an appropriate and timely transition to post-acute services. Communicates initiation of new referrals via electronic media to key stake holders.

Communicates quickly and effectively with all referral sources at every stage of the referral process as a means to achieve referral source satisfaction and to advocate for patients by providing a prompt decision regarding admission. Coordinates the pre-admission screening process and collaborates with the Medical Director in reviewing screened patients for potential admission. Educates physicians and discharge planners on scope of services that Loyola Post-Acute Rehabilitation can provide. 2 5% Yes Performance Improvement- Assists Rehab management staff in identification of opportunities for improving intake operations and scope of services that are provided. 3 5% Yes Case management- Provides back-up support for initial authorization and on-going utilization review for patients with private insurance4 20% Yes Communication and marketing- Maintains positive relations with key referral sources from within and outside of the health system.

Serves as a point of contact for referral sources / providers needing to provide current orders/referrals for patient assessment. Maintains referral source contacts. Tracks and reports referrals by source, payer, and conversion.

Receives and responds to reported intake referral concerns. 5 30% Yes Central Intake-Serves as the point of contact for referral sources from internal and external facilities for potential post-acute rehab patients. Assists referral source in verifying the clinical appropriateness, the social appropriateness, financial appropriateness, and overall logistics in receiving rehab services from Loyola. Secures all required written documentation to facilitate initiation of Loyola Rehabilitation Services.

Identifies primary rehabilitation diagnosis through clinical chart review. Builds referral on above information. Refers to alternative providers based on patient choice, insurance, scope of services required and demographics

Required: Associates Degree

Preferred: Bachelors Degree
