Intake LPN LVN

The Access Services LPN serves as the main access point for Home Health Services by coordinating prescribed services with the physician, payor, patient/caregiver, and provider to ensure that the services are provided in a timely and coordinated fashion. Additionally, this position reviews all patient referrals for appropriateness and compliance with Home Health Conditions of Participation, federal, state and local rules and regulations.

Required Qualifications:

• Graduate of school nursing to obtain LPN
• Upon hire: Washington Licensed Practical Nurse
• 2 years Nursing experience.

Preferred Qualifications:

• 1 year Experience in home health nursing.
