JOB SUMMARY AND PURPOSE

Under the direct supervision of the COO, the Intake/Discharge Registered Nurse (RN) provides professional nursing services organization wide, providing guidance and supervision over medical screening of clients referred to VOALA shelters or residential programs. Acting both as a medical liaison and as a patient advocate, the RN will provide an intake assessment for all referred homeless clients for VOALA programs and ensure clearance an the best placement for clients. In addition, the Intake/Discharge RN will work with primary healthcare providers determining the appropriate level of care and location for sick clients to be discharged to. The RN will also work closely with a social services Case Manager to determine the social service needs of the client.

DUTIES AND RESPONSIBILITIES

  • Provides intake and discharge nursing services including: direct communication with medical providers, referrals to other provides and community health resources, general health instruction to clients, placement to VOALA shelters according to protocols after clearance, and instruction and in-servicing to shelter/counseling line staff including instruction on continued monitoring of clients accepted into VOALA shelters/programs.

  • Responsible to complete a health intake for homeless and other clients referred to VOALA for shelters and/or residential sites or day programs.

  • Based on parameters provided by key leaders, determines the appropriate placement for the referred client

  • If the client does not meet the criteria for placement, works with partnering referring organization/individual to locate medical care facility for treatment prior to admission to program(s)

  • Responsible to collect data on intakes and referrals for tracking purposes

  • Manages and tracks discharges from shelters, when discharges are for health reasons

  • Determines appropriate health care provider to send sick clients after they have been discharged from the program for medical care.

  • Documents patient's current medication list, including: name of prescriber, names of medication, dosages, frequency, duration and start date, trains staff on monitoring client self-administration of prescribed medication

  • Works collaboratively with social services Case manager to determine appropriate social services care

  • Participates in weekly case review with Program Staff to ensure best quality care for clients, and continuous improvement of services.

  • Acts as liaison between Program Staff and Medical Providers, in order to coordinate best care of clients

  • Maintain electronic health records and hard copy documentation of all services provided

  • Other Tasks that may be required to support the Program Managers

Qualifications

REQUIREMENTS:

  • Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications

  • Valid California driver's license, acceptable proof of automobile insurance, and/or reliable mode of transportation

EDUCATION:

  • Current and valid California License Registered Nurse

  • Valid Cardio Pulmonary Resuscitation (CPR) certification

EXPERIENCE:

  • 2 years of work experience as a Registered Nurse

KNOWLEDGE

  • Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA

TECHNOLOGY SKILLS:

  • Microsoft Office (Outlook, Word, Excel)
  • Familiar working with Electronic Health Records

SKILLS AND ABILITIES:

  • Ability to perform under circumstances of possible emotional stress and conflict, including dealing with un-cooperative clients.

  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state, and local standards, with or without reasonable accommodation, including meeting qualitative and quantitative productivity standards.

  • Ability to maintain regular, punctual attendance consistent with ADA, FMLA, and other federal, state, and local standards.

COMPETENCE:

  • Ability to administer tuberculosis (TB) skin test and providing results

  • Excellent communication skills at level necessary for taking patients' medical histories, understanding provider and supervisor instructions, and for accurately documenting patients' medical information

  • Ability to effectively communicate with patient population and staff will demonstrating a high degree of diplomacy and tact

PREFERRED QUALIFICATIONS:

  • Bachelor's degree in Nursing, Nurse Science, or Public Health

  • Culturally sensitive and demonstrated ability and effectiveness working with mental health, substance use disorder, and veterans

  • Experience working with the homeless population

  • Bilingual in English and Spanish
