We want you on our team...Bourbon Community Hospital located in Paris, a 20 minute drive from Lexington, KY is searching for an Intake Coordinator to join Stoner Creek Behavioral Health in performing a variety of duties both clinical and clerical in nature; plans organizes, and coordinates the delivery of patient care by all staff personnel as well as the coordination of day to day office activities.

Essential functions of position include, but are not limited to:

  • Receives and coordinates all referrals to include notification of insurance providers for prior authorization and services covered

  • Responsible for the review of daily logs and notes from Case Managers to assure continuity of care, documentation of nursing process, appropriateness of intervention/action and timeliness of documentation

  • Using discretion and independent judgment in handling patient or physician complaints, receives documents and forwards to appropriate administrative staff

  • Responsible for maintaining supply inventory as well as daily distribution of supplies to field staff. Maintains log and any reports essential for appropriate billing and reorder process

  • Communicates with Department Director any information received from physician, patent or employees about the patent's care or needs

  • Primary contact for receiving phone calls to Stoner Creek

  • Takes physician orders by phone, documents, notifies appropriate Case Manager and adjusts schedules accordingly as indicated. Files all office records as necessary

  • Provide direct patient care on an as-needed basis, as assigned by Department Director

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action

Job Requirements

Education:

  • Associate's Degree, required

  • Bachelor's Degree, preferred

Licensure/Certifications:

  • Valid current Kentucky RN state licensure OR
  • Valid current Kentucky LPH state licensure OR
  • Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW)
  • NCI

Distributes and explains forms, documents, and educational handouts to patients or family members. Meets with patient or patient's caregiver before or after admission to exchange necessary information and documentation. Provides explanation of process and addresses concerns and questions. Communicates with admitting physician's office, nursing unit staff, and/or other appropriate personnel regarding admission to exchange necessary information and determine placement. Verifies insurance benefits and obtains precertification/authorization as necessary. Determines and accepts required payments, including co-pays and deductibles, or refers to financial counselors for follow up.
