Intake Coordinator (RN) - Full Time

SandyPines is a 149 bed Residential Psychiatric Facility servicing children and adolescents ages 5 to 17. The facility is located in beautiful sub-tropical South Florida and is nestled beside Jonathan Dickinson State Park which creates a rustic, roomy outdoor atmosphere. SandyPines is at the county line of both Palm Beach and Martin Counties and is just minutes away from the shores of the Atlantic Ocean.

Job Description

The Intake Counselor/RN responds to all persons requesting assistance either by telephone or in person, and provides consultation about medical questions and concerns to staff with MD for the intake counselors. They will also complete nurse admissions paperwork and vitals as needed. Other duties include, but are not limited to; scheduling patient assessments/appointments; creating and updating patient pre-registrations including verification of benefits and financial resources; setting certain expectations for patients regarding their services, reviewing transfers, and responding to customers' information needs. Admissions to the Hospital are facilitated through coordinated efforts with the Business Office, Utilization Review and the Department of Nursing Services.

Essential Functions

  • Timely Data Entry in MS4 system for incoming and discharging residents

  • Conducts tour of the facility for family and new residents

  • Chart audits for COW as needed

  • Provides backup for Admissions Assistant and Director of Admissions.

  • Responds to Dr. Martins

  • Returns phone calls in a timely manner

  • Ensures effective communication with customers and their companions that are deaf or hard-of-hearing

  • Assists the department with scheduling admissions and notifiying parents/guardians of items needed for admission

  • Assists in completing Admissions packets and organizing files in the Admissions drive

  • Assists in completing pre-certifications and gathering clinical documentation on residents

  • Completes Admissions Assessments and required documentation in a timely manner

  • Assists in maintaining the Engagement List and in scheduling of admissions

  • Maintains positive relationships with referral sources and responds to their requests in a timely manner

  • Completes vitals and other pertinent paperwork with the patient

Requirements

Florida licensed Registered Nurse or Masters' Degree in a Social Services related field with experience in completing biopsychosocial assessments. Excellent organizational, computer and written and oral communication skills are also required.

Communication Skills Ability to comprehend and give correct written instructions. Ability to communicate verbally with customers and co-workers to process orders both over the phone and in person. Essential Functions/Skills Ability to add, subtract, multiply and divide accurately and quickly (may use calculator). Must be able to make correct monetary change. Verbal, writing, and telephone skills to take and process orders. Motor coordination between eyes and hands/fingers to rapidly and accurately make precise movements with speed. Ability to enter orders using a computer keyboard or touch screen. Work Conditions EXPOSURE TO: Varying and sometimes adverse weather conditions when removing trash and performing other outside tasks. In-store temperatures range from 36 degrees in cooler and 90 degrees and above in some work areas. Sudden changes in temperature in work area and while outside. Fumes from food odors. Exposure to cornmeal dust. Cramped quarters including walk-in cooler. Hot surfacdes/tools from oven up to 500 degrees or higher. Sharp edge and moving mechanical parts. SENSING: Talking and hearing on telephone. Near and mid--range vision for most in-store tasks. Depth perception. Ability to differentiate between hot and cold surfaces. TEMPERAMENTS: The ability to direct activities, perform repetitive tasks, work alone and with others, work under stress, meet strict quality contro standards, deal with people, analyze and compile data, make judgements and decisions. STANDING: Most tasks are performed from a standing position. Walking surfaces include ceramic tile"bricks" with linoleum in some food process areas. Height of work surfaces is between 36" and 48". WALKING: Walking is generally in short distances for short durations. SITTING: Paperwork is normally completed in an office at a desk or table. LIFTING: Bulk product deliveries are made twice a week or more and are unloaded by the team member using a hand truck. Deliveries may include cases of ingredients and supplies weighing up to 50 pounds with dimensions of up to 3' x 1.5'. Cases are usually lifted from floor and stacked onto shelves up to 72"high. CARRYING: Large cans, weighing 3 pounds, 7 ounces, are carried from the workstation to storage shelves. Occasionally, pizza sauce weighing 40 pounds is carried from the storage room to the front of the store. Trays of pizza dough are carried two at a time over short distances, and weigh approximately 12 pounds per tray. PUSHING: Pushing is performed to move trays which are placed on dollies. A stack of trays on a dolly is approximately 24"-30"and requires a force of up to 7.5 pounds to push. Trays may also be pulled. CLIMBING: Team members must infrequently navigate stairs or climb a ladder to change prices on signs, wash walls, perform maintenance. STOOPING/BENDING: Forward bending at the waist is necessary at the pizza assembly station. Toe room is present, but workers are unable to flex their knees while standing at this station. Duration of this position is approximately 30-45 seconds at one time, repeated continuously during the day. Forward bending is also present at the front counter and when stocking ingredients. CROUCHING/SQUATTING: Performed occasionally to stock shelves and to clean low areas. REACHING: Reaching is performed continuously; up, down and forward. Workers reach above 72"occasionally to turn on/off oven controls, change prices on sign, and lift and lower objects to and from shelves. Workers reashing down to perform such tasks as scooping cornmeal from a plastic barrel, or washing dishes.
