The Intake Coordinator for the Geriatric Psychiatric Unit holds a degree in psychology or social work and has experience working with the geriatric psych population. He/She works with the patient care team to facilitate admissions into the inpatient setting.
The Intake Coordinator reports directly to the Chief Nursing Officer but must work collaboratively with all members of the patient care team to ensure admissions to the unit are done in an appropriate and efficient manner. He/She has a degree in either psychology or social work and must have the skills necessary to provide therapeutic social services under the direction of a Master's prepared Social Worker (MSW) to patients on the unit. This may include completing the initial intake assessment form, coordinating the admission process, and coordinating with physicians, advance practice nurses, nurses, families and patients so the transition into the inpatient setting is uncomplicated.
The Intake Coordinator may be responsible for the initial assessment of the patient prior to entering the Geriatric Psych Unit. Other Social Service activities that the Intake Coordinator may be asked to do include biopsychosocial assessments, consents for treatment, daily group and individual sessions, and discharge planning. The Intake Coordinator for the Geriatric Psych Unit provides marketing activities for the service line which includes visiting referral and potential referral sources, providing information about the services provided within the unit and facilitating referrals from those entities
Must possess a degree in Psychology or Social Work with an active license in the state of TN
Minimum of 2yrs clinical experience, preferably with geriatric patients
Must have current BLS certification.
