Moda Health is seeking an Intake Coordinator. The Intake Coordinator is responsible for processing information about new patients.

Primary Functions:

Intake information from referral sources and patients in a professional, knowledgeable, and courteous manner.
Route patient referrals to the appropriate department or individual(s) in the pharmacy.
Establish and maintain accurate and complete patient records.
Receive calls from new patients and referral sources and document all customer communications.
Assist billing department with the verification of insurance coverage for new customers.
Assist billing department with the re-verification of insurance coverage for current customers.
Prepare and secure medical necessity documents.
Communicate with billing department to ensure proper billing to insurance companies, contracts, other third-party payers and patients for all products and services delivered; maintain accurate billing records.
Report any misconduct, suspicious or unethical activities to the Compliance Officer.
Participate in surveys conducted by authorized inspection agencies
Participate in the pharmacy's Performance Improvement program as requested by the Performance Improvement Coordinator.
Participate in pharmacy committees when requested.
Participate in in-service education programs provided by the pharmacy.
Perform other duties as assigned by supervisor.

Clinical Sales Liaison will work directly with case managers and discharge planners to process home infusion referrals within the hospital systems. This position requires sales training as directed by Director Regional Sales. Primary Functions: 01. Provide clinical education and manage marketing services to clinic's, hospitals and physician referral sources. 02. Assess and coordinate, in partnership with Case Managers and Discharge Planners, home infusion services to assist with determining the plan of care administered by Pharmacy Infusion Services under the direction of the patient's provider. 03. Conducts patient assessments to identify patients for potential admission into the system. 04. Assist with providing patient and caregiver training on therapy, monitoring, and side effects; demonstrate procedure for administering therapy in the home, with a return demonstration from the patient/caregiver 05. Maintain and grow facility customer relationships and provide responsive customer service with assigned accounts; evaluate current and future clinical and service needs for existing and new accounts 06. Conduct in-services and continuing education programs for referral sources and/or their support staff; partner with Regional Sales Director on presentations to current and potential accounts 07. Review claims denied for payment or underpaid; resolve discrepancies and resubmit as needed. 04. Post credit for returns, or overpayments; process refunds as needed. 05. Pursue timely collection of unpaid overdue balances; coordinate with collection agencies as needed. 06. Maintain record keeping so that all information is accurate and complete. 07. The enteral coordinator will support enteral/parenteral nutrition patients by coordinating supply and formula provisions in an accurate and timely basis. Maintain a positive working relationship with all related nutrition staff to maximize the effectiveness of the nutrition program. Primary Functions: 01. Manage incoming calls from referral sources to initiate or discontinue enteral services. 02. Respond to incoming calls from patients, caregivers, and health care providers regarding patient enteral supply needs. 03. Collaborate with clinical liaisons, discharge planners and other referral sources to coordinate care for prospective enteral patients. 04. Confirm insurance and benefit eligibility on all new referrals. 05. Perform monthly verification of coverage for long term Medicaid patients. 06. Create and review the Cost Disclosure/Advance Beneficiary Notice for Medicare patients and track signed forms. 07. Generate and review benefits letter to inform patient of financial responsibility. 08. Coordinate scheduling and delivery of supplies. 09. Perform monthly monitoring of enteral nutrition including any problems or non-compliance to the appropriate staff member. 10. Frequent review of supply inventory and provide patients with supplies based on established guidelines. 11. Communicate with appropriate staff member regarding feeding pump needs or problems. 12. Ensure accurate tracking of nutrition therapy equipment, schedule enteral pump swaps for annual maintenance as required by ACHC and as needed. 13. Contact patients at completion of therapy to arrange for equipment pic up. 14. Maintain current enteral orders, track expiration dates and contacts physician for new or updated orders as appropriate. 15. Observe legal and ethical guidelines for safeguarding the confidentiality of patient information. Are you ready to be a betterist?
