LOCATION: Los Angeles, Ca

CLASSIFICATION: Non-Exempt, Full-Time

SALARY: $20.00 Hourly Rate

  • Work Shift Tuesday - Saturday, 9AM - 5:30PM

Intake Coordinator

The Intake Coordinator will manage the referral process serving as a liaison between the hospital social workers and the NHF Recuperative care staff, ensuring a smooth transition from hospital to our Recuperative Care Program. They will assure information is gathered from hospitals accurately and thoroughly according to established program policies. They would be tasked at fostering a positive and collaborative partnership with our partners, providing quality customer service and clear and effective communication.

Responsibilities and Initiatives

To help NHF meet its growth goals, the Intake Coordinator will:

  • Serve as point of contact for partners on referrals, providing internal and external updates on referral status. Check and maintain bed reservations, where applicable.
  • Ensure accuracy and completion of referral packet received. Retrieve and ensure referrals are entered in a timely manner into a database, confirming partners have received a response within a short time and finalizing referral by letting the facility know that a guest will be arriving.
  • Create and initiate guest files in both NHF CRM database. Maintain confidentiality of patient information.
  • Communicate effectively with team about referrals and admissions.
  • Build trusting relationships with partners and guests. Deliver excellent customer service and exercise good judgment with a compassionate, positive attitude while managing a population of guests who may be experiencing difficult life events, such as homelessness, unemployment, addiction recovery, health issues and/or poverty.
  • Provide input and evaluation of programming for quality improvement purposes.
  • Work collaboratively with other members of the team, including active participation in department and agency staff meetings and training. Assist in providing information and support to the team.
  • Project a positive professional image and adhere to organization dress code.
  • Flexibility to adapt to schedule changes to meet the needs of the facility.
  • Must be able to aid other NHF facilities as assigned by supervisor when the need arises.
  • Complete additional tasks as assigned to meet the needs of the facility, notwithstanding primary roles.
  • Always follow NHF policies and procedures.

Requirements

  • At least 2 years of experience in an administrative support capacity, 1 year of experience in a healthcare or social services setting.
  • Strong proficiency with Microsoft Suite which includes Word, Excel, and Outlook.
  • Have strong written and verbal communication skills.
  • Excellent in multi-tasking; have strong organizational and time management skills.
  • Experience in medical office/recuperative care setting (preferred).
  • Familiarity with HIPAA (preferred).
  • Have friendly and service-oriented mindset. Be people first and be driven by empathy.
  • Be comfortable in a fast-paced environment, 50+ bed recuperative care facility. Ability to remain calm in escalated situations.
  • Bilingual in a language that is commonly spoken in Los Angeles, other than English. (Preferred, not required).

Physical Requirements

  • Works in office area under normal working conditions.
  • Is subject to frequent interruptions.
  • Must be able to read material and use a computer.
  • Must be able to hear well enough to communicate with co-workers.
  • Standing and walking.
  • Climbing/Stooping/Kneeling.
  • Lifting/Pulling/Pushing.
  • Fingering/Grasping/Feeling.
