You'll manage the entire intake process, provide transitional support, and conduct annual level of care assessments.

Your clinical expertise and compassionate approach will make a real difference in the lives of our clients. Key Responsibilities: Intake Management:

Guide individuals through the initial stages of care, ensuring a warm and welcoming experience. Transitional Support: Maintain a caseload of BHH clients, providing crucial support until they are matched with the perfect provider.

Level of Care Assessments: Apply your clinical skills to assess client needs and determine appropriate levels of care. Collaboration: Work closely with our BHH team, sharing your insights regarding clients needs, barriers and other necessary information to enhance client care.
