As an Intake Assessment Counselor, you will provide mental health and substance abuse intake, assessment, and referrals. This position is the gatekeeping role for client and others to access our Caring for Caregivers services. Our team helps others recognize that it is okay to ask for help and make it as seamless as possible. Your duties will include but are not limited to: • Responding to crisis situations by conducting primarily phone assessments. • Establishing and maintaining relationships with external physicians and other hospital personnel. • Performing clinical and diagnostic assessments for patients of all ages. Job Responsibilities:

  • Responds to intake calls and emails with sense of urgency and care.

  • Provides intake services including brief assessments, scheduling, information and referrals.

  • Offers consultation, crisis intervention, and case management services.

  • Documents all information necessary to establish appropriate care and follow-up.

  • Provides brief, solution-focused counseling to caregivers and eligible household dependents.

  • Establishes and maintains relationships with internal team members, organizational stakeholders, and external providers to promote quality and continuity of care.

  • Participates in continuing professional education, departmental meetings, and case consultation toward professional growth and development.

  • Other duties as assigned.

Education:

  • Bachelor's degree in a behavioral health field required.

  • Master's degree in a behavioral health field preferred.

Certifications:

  • Licensed Social Worker (LSW) required OR Licensed Professional Counselor (LPC) required.

  • Licensed Independent Social Worker (LISW) OR Licensed Professional Clinical Counselor (LPCC) preferred.

Complexity of Work:

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

  • Must be able to work in a stressful environment and take appropriate action.

Work Experience:

  • Minimum one year experience in clinical setting.

  • A one year internship in a clinical setting may be considered.

  • Two years of experience in a clinical setting is preferred.

Physical Requirements:

  • Sedentary Work - Exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3 of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body.

  • Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time.

  • Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required.
