In accordance with the standards of care established by the State Board of Nursing, Responsible for the delivery of patient care through the nursing process of assessment, planning, implementation and evaluation. Directs and guides patient teaching and activities of ancillary personnel.

Job Duties

  • Provides direct patient care including, treatments, medications, and diagnostic studies according to policy and procedure.

  • Completes assessments as established by scope of practice and unit standards.

  • Addresses physical, psychological, social, cultural, and spiritual needs of patient and family throughout the continuum of care.

  • Formulates, coordinates, and provides plan of care as established in unit standards including, patient and family education, family needs and discharge planning throughout hospitalization with members of the interdisciplinary team.

  • Evaluates effectiveness of nursing interventions and revises plan of care based upon patient response to interventions.

  • Initiates collaboration with physicians, ancillary personnel, and other interdisciplinary team members and takes action to proactively resolve and facilitate patient needs.

  • Makes assignments in collaboration with other team members.

  • Directs and alters assignments in response to changes in patient situations.

  • Promotes teamwork by delegating tasks to team members based on skill, expertise, time availability, acuity, and needs of the patient.

  • Problem solves, troubleshoots and takes corrective measures in the care and handling of equipment.

  • Participates in unit-based decision-making regarding the utilization of evidence based practice.

  • Actively participates as a team member working toward team goals of the unit and organization and is accountable for unit and system specific metrics.

  • Supports and participates in initiatives that support the Magnet mission.

  • Actively participates in patient quality and safety education programs and other activities designed to improve departmental and organizational quality and safety.

Med Surg-Oncology / 40 hours per week / 7a-3:30p / every other weekend and holiday The primary focus of the Nurse Aide is to perform simple patient care functions within their level of competence under the direct supervision of the RN. * Current CPR certified * At least one year experience in patient care preferred Responsible for maintaining a safe environment of care. Practices Compassionate Care Model. Answers patients' call lights in a timely manner. Observes patients' conditions, measuring and recording intake and output and vital signs, patient weights and reports changes to professional staff & documents results. Performs hourly rounding routinely. Cleans rooms and change linens, as needed. Demonstrates ability to prioritize tasks in order to complete assignments in a timely fashion. Performs other related duties as assigned by the Registered Nurse for efficient operation of the unit and care of patient. Supports and adheres to all unit, nursing and hospital policies/procedures. Participates in staff orientation on assigned shift and provides ongoing feedback regarding new employees' performance. Maintains and ensures security and safety of all equipment and sup plies on the unit and takes necessary corrective action as needed. Performs patient care skills including personal hygiene, ADL (feeding, ambulation and repositioning) and perform specific procedures associated with elimination. Follows safety measures including isolation precautions, fire/safety/disaster policies. Performs collection of patient specimens, vital signs and patients weights.
