The Care Manager partners with, assesses, and plans interventions to help patients/families cope with social, emotional, economic, and environmental problems.
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Consults with multidisciplinary teams regularly to evaluate patient's status and appropriateness of medical care, including coordination of care.
Participates in discharge planning including identification and coordination of appropriate resources. Provides discharge education and resource referrals to patients when necessary.
The ideal candidate will have:
Bachelor's degree in Social Work or Nursing preferred.
Experience with post-acute care or skilled rehab.
Excellent organizational skills and ability to prioritize work in a fast paced environment.
Demonstrated ability in managing crucial or difficult conversations.
Maine state licensure as Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or Registered Nurse (RN) required. Certification as a Case Manager (CCM) by the Commission for Case Manager Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association (ACMA) preferred.
