The Infusion Nurse will be responsible for delivering care and an excellent patient experience for Rheumatology patients. The Infusion Nurse will perform assessments, provide evaluation and create a plan of care in coordination with the Rheumatologists. Other duties will include infusion therapy, triage, clinical care coordination with the healthcare team.

Typical hours are M-F 730am-5pm, scheduled 3 days (24 hrs) per week, days vary depending on business needs.

Primary location is at Woodbury Clinic and will work at the Hudson location as needed.

Essential Functions:

  • Managing and monitoring the IV center:

  • Performing blood draws

  • Completing IV infusion and monitoring multiple patients

  • Implement Treatment plans: evaluate patient progress towards goals and educate patient and treatment plan / progress

  • Nursing Functions:

  • Follows and implements designated policy and procedures.

  • Demonstrates clinical competency, and a holistic approach to patient care, caring for the whole patient.

  • Holistic Care Coordination:

  • Caring for the whole patient to ensure support of the family and patient throughout the care continuum. This includes assisting the patient and family navigation of the care system in order to provide optimal care to meet the needs of the patient.

  • Collaborates with the healthcare team to ensure optimal patient outcomes and results.

  • Ability to establish rapport with the patient to work on shared goals and priorities.

  • Documentation:

  • Ensuring accurate clinical care is documented in the EMR.

  • Collaboration:

  • Collaborates with the care team around essential operational functions such as patient referrals, clinical care coordination with the care team and physicians to include out-patient settings.

  • Other duties as assigned

Education and Experience Requirements:

  • Graduate of an Accredited School of Nursing

  • Current RN licensure in Minnesota or Wisconsin - depending on clinic locations

  • Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained.

Preferred Requirements

  • One-year full time employment as an RN

  • Rheumatology experience

  • Infusion Therapy experience

  • Ability to work independently with minimal supervision.

  • Independent travel to other work locations, if needed.
