Bachelor's Degree in nursing preferred. Graduate of accredited diploma or AD Program may be considered with demonstrated evidence of professional growth and development and as permitted by state regulation required. Minimum of 1 years nursing experience, home infusion experience preferred.
Certificates and Licenses:
Current state licensure as a registered nurse required.
CRNI certification preferred.
CPR certification required.
Must have a valid driver license
Must have access to a dependable vehicle with the minimum auto insurance as required below:
$250,000/$500,00 bodily injury
$100,000 property damage OR
$300,000 combined single limit (CSL)
Duties and Responsibilities include the following. Other duties may be assigned.
Performs direct and indirect care to patients in the home or alternate setting.
Develops and ensures appropriate distribution of consumer/patient educational materials.
Assess patients during initial visit for current medical and psychosocial needs. Develop plan of treatment and plan of care.
Completes care conferences as assigned by DON.
Delivery of medications and supplies as requested.
Discharges patients per policy.
Coordinates lab draws and delivery.
Performs well independently and solve problems.
Communication to pharmacy regarding patient supplies/medication inventory.
Provides telephonic and face-to-face consumer educational services.
Confers with customers (physicians, patient/family, health care personnel, etc) during the planning process to establish mutual goals.
Document in computer software system.
Maintain adequate nursing supplies in nursing bag and to be available at every nursing visit.
Provide on-call coverage and participate in the on-call rotation.
Communicates the Patient Bill of Rights and Responsibilities to the patients and caregivers, obtains signatures on the Agreement for Services, consent, and other forms as required by law
Participates as necessary in care conferences and meetings related to DSIG patients.
Program and tool development for DSIG programs.
Timely and appropriate documentation as indicated.
Local and regional representation with non-profit organizations.
Keeps current regarding community health issues and regulations.
Development of educational programs and symposiums locally and regionally.
Completes required reporting and accounting of time and expenses within the required timeframe.
Demonstrates a high degree of personal responsibility regarding accuracy and quality of work.
Compliant with all State, Federal and Local regulations regarding nursing practice and the provision of patient care.
